Home Health Physical Therapist - PRN

Prioritize workload so as to meet productivity standards and applicable deadlines.

Verify and interpret physician referrals for PT meeting current documentation standards and policies. Provides initial and ongoing assessments to determine physical therapy needs, including OASIS assessment at appropriate time points.

Performs appropriate assessments on each patient as related to the Physical therapy Plan of Care created individually & carried out at home according to treatment goals and interventions that focus on musculoskeletal, neurological, and functional ability testing

Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environment, and infection control standards. Participates in performance improvement projects for the department and organization's CQI activities.

Communicates/ consults appropriately, professionally, and clearly to physicians, supervisors, staff, and other departments coordinating with other team members as needed.

Maintains records pertinent to the personnel and operation of the department.

Supervises/delegates to therapy assistants and home health aides as appropriated.
