We are seeking a Physical Therapist for a full-time and 100% Home Health focused opportunity in Fort Collins, CO!

Compensation Package:

• $57 to $65 Per Regular Visit 
• $80 to $95 Per Non Medicare Visit 
• $95 to $115 Per Medicare Visit
• $80 to $95 Per Evaluation Visit

Requirements:

• Clear and Active PT License in Colorado
• New Graduates Considered
• Current and Active Driver’s License
• Home Health Experience Preferred- Training Offered

Responsibilities:

• Monday through Friday 8 AM to 5 PM
• 1 Saturday or Sunday Every 4 to 6 Weeks for Additional Compensation OR 1 Day Off
• Working with Geriatric Patients 
• Using Matrix Care EMR and OASIS Documentation- Training Offered on Both 
• Seeing 5 Geriatric Patients Per Day in Their Home- Routes Are Coordinated Depending on Your Home Location
• 1 to 2 Weeks of Onboarding/Orientation
• Comply with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
• Assess each patient and developing an appropriate treatment plan with measurable goals and time frames.
• Demonstrate treatment skills that are rooted in evidence-based practice guidelines while balancing both patient and provider expectations and needs
