Responsibilities:

• Conduct comprehensive evaluations of patients' physical abilities and limitations

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and overall functional independence

• Provide skilled therapeutic interventions, such as therapeutic exercises, manual therapy techniques, and assistive device training

• Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies

• Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care

• Document and maintain accurate records of patients' progress and treatment plans

Qualifications:

• Active and unrestricted Physical Therapist license in the state of Florida

• Minimum of 1 year of experience in physical therapy, preferably in a home health or a related setting

• Strong clinical skills and ability to provide patient-centered care

• Excellent communication and interpersonal skills

• Ability to work independently and make sound clinical judgments

• Proficiency in electronic medical record (EMR) systems

• Valid driver's license and reliable transportation to travel to patients' homes.
