This position requires collaborative patient care delivery and exceptional service in line with industry benchmarks throughout West Michigan. This role offers a comprehensive rewards package and a supportive work environment. Key Responsibilities:

• Evaluate and implement patient care plans in a home health setting.

• Monitor and document patient progress.

• Educate patients and caregivers on therapy and device usage.

• Supervise therapy assistants and coordinate with the care team.

• Serve as an educational resource for the clinical team.

• Ensure adherence to patient safety and care standards.

• Manage time effectively and meet documentation deadlines.

• Utilize technology, such as tablets and EMR software, competently. Qualifications: - Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.

• Valid Physical Therapy license in Michigan.

• Minimum of one year of healthcare experience.

• Current driver’s license and comfort with local travel.

• Ability to consistently meet critical charting deadlines. Work Schedule: - 40 hours per week, with flexible scheduling.

• Some travel to patient homes in Benton Harbor and surrounding areas. Rewards Package: - $10,000 success bonus in the first year, paid in installments.

• An additional $5,000 bonus to assist with relocation for qualified candidates.

• Pay-per-visit compensation model with rates based on service type and experience.
