Alpine Home Health Care is actively seeking Registered Nurses, with a background in Pediatric care, who want to provide care to clinically complex patients in a home setting must be able to case manage. Immediate openings for Full-Time Field RN's in Bronx, Queens, Brooklyn and Manhattan. One year of inpatient experience preferred. Prior home care experience required.

Responsibilities Include:

  • The RN Care Coordinator is a field-based nursing position

  • Perform patient assessments in order to implement best Plan of Care

  • Coordinate resources & services for assigned case load; working with the patient, family & physician

  • Maintain up-to-date / accurate documentation for assigned cases

  • Maintains a cooperative relationship among health care teams

  • Bilingual Field Nurses a BIG Plus!

  • English / Spanish

Requirements Include:

  • Valid NYS Registered Nurses license

  • Minimum 1 year home care experience

  • Relevant CHHA experience a BIG Plus

  • Solid computer skills; working knowledge of MS Office

  • Experience with Electronic Medical Records preferred

Position: Senior Coordinator Home Health (Must be bilingual in Spanish/English) Reports to: Coordinating Manager Job Description/Requirements: As a Senior Coordinator, you will: * Assign and schedule a Home Health Aide to each patient referral for which he/she is responsible for. * Appropriately supervise the Home Health Aides assigned to his/her patients to ensure quality of service. * Appropriately manage their assigned patients and communicate all pertinent information with other Alliance staff / Service provider as may be necessary (e.g. HHA, clinical staff, management, contract vendor etc.) in a timely fashion. * Be responsible for resolving issues related but not limited to patients managed by other Coordinators. * Report any unusual circumstances to the Vice President of Operations immediately. * Adhere to all Alliance and individual Contract Vendor Policies and Procedures. * Adjust Home Health Aide (HHA) assignments as may be necessary for them to fulfill their annual regulatory requirements. * Assign and schedules appropriate Home Health Aides to patients as required in a timely manner. An uncovered patient is a violation of company policy and disciplinary action, including termination, may result. * Provide accurate and timely communication regarding patients to the appropriate parties based on the situation. * Provide the patient and/or family member the name of the scheduled caregiver (or replacement caregiver) in advance. * Supervise Home Health Aides and refers any patient and/or Home Health Aide issue or incident requiring investigation and follow-up to the Internal Investigations Department, following established methods of communication and documentation. * Supervise Coordinators, and Coordinator Floats. * Fulfill "On-call" responsibilities as required. * Maintain confidentiality of all patient information. * Monitor and resolves daily Staff Monitor Time and Attendance Alerts. * Prepare and conducts timely Home Health Aide Administrative Evaluations. Requirements of Qualified Candidates: (Must be bilingual in Spanish/English) * High School Diploma or equivalent required * One year of related experience and/or training preferred * Ability to read and interpret documents and procedure manuals among others * Ability to write routine reports and correspondence * Ability to speak effectively in clear, professional voice * Strong telephone and organizational skills required * Ability to apply common sense understanding to carry out instructions furnished in written or oral form * Ability to deal with problems involving several concrete variables in standardized situations * Ability to identify and resolve problem in a timely manner; gather and analyze information skillfully; develop alternative solutions; work well in group problem solving situations; and use reason even when dealing with emotional topics * Ability to manage difficult or emotional customer situations; respond promptly to customer needs; solicit customer feedback to improve service; respond to requests for service and assistance; and meet commitments AccentCare, Inc. company is one of the largest home care companies in the United States.
