Home HealthMedical Social Worker Responsibilities:

Assist client and family in understanding, accepting, and following, medical recommendations and provide services planned to restore the patient to optimum social and health adjustments within patient's capacity
Assess the patient and family with personal environmental difficulty in order to estimate their capacity and potential to cope with problems of daily living.
Participate in the development of and makes suggestions to the physician for additions to the Plan of Care.
Provide supportive casework to assist in restoring the patient to his/her optimum level of social health adjustment.
Work with family to assist in utilization of appropriate community resources or to live within the disability.

Home HealthMedical Social WorkerQualifications:

Must be able to comprehend written/verbal communication.
Must be able to use good and sound clinical judgment.
Must be able to document accurately and appropriately.
Must follow up as needed and appropriately with doctor's office, RHH clinical and management staff pertaining to any concerns regarding patient care
Master's degree from a school of Social Work that has been approved by the Council of Social Work Education.
Must hold, in good standing, a Licensed Social Worker (LSW), issued by the State of Illinois. Licensed Clinical Social Worker (LCSW) preferred.
One (1) to two (2) years post graduate social work experience in a health care setting.

The position is responsible for working collaboratively with local/VISN leadership to identify and/or implement best practices, advise Leadership of all program requirements pertaining to the C&P Program and serve as the VA's Senior Technical Advisor and as the primary consultant & liaison to the VISN C&P Officer. Learn more about this agency Responsibilities The incumbent oversees all functions related to healthcare providers' professional credentials, supports all divisions of the VAMC (i.e. full time, part time, temporary, intermittent, contract, without compensation (WOC) and fee basis) in accordance with VHA Credentialing and Privileging Directives, VA Handbooks, Policies, Memos, and Notices; The Joint Commission (TJC) Standards and State and Federal Regulations. The C&P Manager is organizationally aligned to and supervised by the Chief of Staff. The incumbent is responsible for ensuring that employees are efficiently and effectively accomplishing quality work that is compliant with regulatory requirements and procedures, TJC standards, laws, and VHA C&P policies and requirements, as well as assuring implementation and accomplishment of the goals and objectives of the C&P Program. * Provide consultation and guidance to local leadership and C&P employees on all matters pertaining to the facility's C&P program including Healthcare Professionals' credentialing and privileging; facility Scope of Practices; National and VISN Memorandums of Understanding (MOUs); VA Memorandums; VHA requirements, Handbooks, Directives and Policies; TJC Standards; quality assurance assessments; and risk management and malpractice claim action evaluations. Determine appropriate actions, including those resulting from VISN and local level reviews, initial and re-credentialing processes, including, but not limited to primary source verifications; reporting and data trends, identified issues that require further review, and electronic credentialing database and clinical privilege maintenance. * Attend committee meetings and presents providers' credentials, clinical practice histories and red flags; documents and composes committee minutes, privileging requests and/or actions, malpractice payments, tort claims and adverse actions, Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE) reviews and completion timeframes. * Serve as the primary consultant for the facility's C&P program; analyzes program audit results; interprets facility quarterly report card data; researches provider credentialing requirements; identifies potential privileging issues; drafts facility Medical Staff Bylaws updates and revisions; interprets and explains new regulatory requirements, and VHA requirements, procedures and processes; contributes to development of local policies; ensures program compliance; monitors National Practitioner Databank reporting and queries results; prepares Federal and State Medical Boards reports; tracks State Licensing Board reporting; communicates telehealth provider requirements; manages VetPro access levels and training; assists with Joint Commission surveys, and Office of Inspector General and Government Accountability Office reviews. * Coordinate with Service Chiefs and service level administrative teams for annual Committee(s) review of clinical privileges and scope of practice forms; reviews clinical privileges and scope of practices for accuracy, setting specific indicator and signature appropriations as required for VAMC compliance with Joint Commission Standards; updates clinical privilege forms to reflect procedures added or deleted.
