Home HealthMedical Social Worker Responsibilities:
  • Assist client and family in understanding, accepting, and following, medical recommendations and provide services planned to restore the patient to optimum social and health adjustments within patient's capacity

  • Assess the patient and family with personal environmental difficulty in order to estimate their capacity and potential to cope with problems of daily living.

  • Participate in the development of and makes suggestions to the physician for additions to the Plan of Care.

  • Provide supportive casework to assist in restoring the patient to his/her optimum level of social health adjustment.

  • Work with family to assist in utilization of appropriate community resources or to live within the disability.

Home HealthMedical Social WorkerQualifications:
  • Must be able to comprehend written/verbal communication.

  • Must be able to use good and sound clinical judgment.

  • Must be able to document accurately and appropriately.

  • Must follow up as needed and appropriately with doctor's office, RHH clinical and management staff pertaining to any concerns regarding patient care

  • Master's degree from a school of Social Work that has been approved by the Council of Social Work Education.

  • Must hold, in good standing, a Licensed Social Worker (LSW), issued by the State of Illinois. Licensed Clinical Social Worker (LCSW) preferred.

  • One (1) to two (2) years post graduate social work experience in a health care setting.

Why Join Us as a Home Health RN Case Manager: Here are just a few of the reasons why Residential employees have voted us a Top Workplace year after year: * Average compensation range for a Full Time RN (with a full case load): $78,000 * Personal one-on-one connection with patients * Customized training and mentoring * Employee recognition program * Company vehicle program * Education reimbursement benefit * Growth opportunities * Ongoing education and support * Specialized and exclusive clinical programs * Comprehensive benefit program Home Health Registered Nurse Case Manager Responsibilities: * Provide high quality care in the home by developing a plan of care in collaboration with the patient/caregiver, the patient's physicians, and other care disciplines. * Follow disease management processes and established protocols to ensure the goals determined in the plan of care are met. * Educate patients and caregivers on the management of their illness to encourage compliance and prevent rehospitalizations. * Reassess patients to determine if plan of care needs have changed or if it may be appropriate to recommend other disciplines, advanced care planning, or hospice. * Oversee and document home health aide and licensed practical nurse care plans. * Organize daily schedule by assessing individual patient needs and priorities. * Document in Homecare Homebase (HCHB), Residential's EMR, through a company-provided tablet. Home Health Registered Nurse Case Manager Qualifications: * Must hold, in good standing, a license as a RN, issued by the state in which you work. * A minimum of one (1) years of experience working as a RN including some medical/surgical experience and/or an acute care or home care setting is preferred. * Strong communication and organizational skills and high standard of ethics and compliance. * Ability to document within an electronic medical record. * Associate Degree from an accredited School of Nursing or higher preferred.
