We are seeking a LPN for a full-time and 100% Home Health focused opportunity in the Parker/Castle Rock area, CO!

Compensation Package:

• $30 to $35 Per Regular Visit

Requirements:

• Clear and Active LPN License in Colorado
• 1+ Years of Experience as an LPN
• Current and Active Driver’s License
• Home Health Experience Preferred- Training Offered

Responsibilities:

• Monday through Friday 8 AM to 5 PM
• 1 Saturday and Sunday Every 4 Weeks for Additional Compensation OR 2 Days Off
• Working with Geriatric Patients 
• Using Matrix Care EMR- Training Offered 
• Seeing 6 Geriatric Patients Per Day in Their Home- Routes Are Coordinated Depending on Your Home Location
• 1 to 2 Weeks of Onboarding/Orientation
• Applies nursing knowledge in planning, implementation, and evaluation of human response to actual or potential health problems
• Communicates case findings and initiates health teachings, health counseling, and provision of care that supports life or restores well-being
• Functions under the direction of the Nursing Supervisor
