Contract Pay: $4500/month About A Step Above A Step Above has been serving the Atlanta community for over 4 years. We are a Case Management Agency that assists individuals in receiving services through the Elderly and Disabled Waiver Program (EDWP) through Medicaid. The program is designed to keep loved ones in their homes while receiving services.

Our Case Management Agency currently specializes in enrollment of CCSP and SOURCE qualified individuals. A Step Above Case Management is seeking passionate LPN’s in the Atlanta area who want to make a difference while operating on a flexible schedule. Job Summary Your role will be to schedule six in-home (residence, hospital, long-term care facility, or other appropriate site as indicated) visits per week in a specific county assigned to you with-in Atlanta while conducting Minimum Data Set (MDS) style Initial Assessments for individuals who meet nursing home level of care.

Develop the Comprehensive Care Plan to reduce the risks identified in the MDS-HC triggers. Obtain medical records and any other necessary documentation for member qualifications. Conduct follow-ups on assessment scheduling and re-contacting for missed appointments.

One hour Weekly team zoom meetings regarding updates, progress, comments, questions, concerns, etc. Log assessments and notes into the WellSky program. You will be working under direction and supervision of a licensed professional registered nurse who will review the assessments and your documentation for approval.

Preferred Qualifications: Possess a valid LPN license in the state of Georgia Minimum Data Set (MDS) Assessment Training Preferred Skills Ability to effectively coordinate and communicate with clients, service providers, general public, and other staff members Skill in establishing and sustaining interpersonal relationships Knowledge of human behavior, gerontology Skills in team building and group dynamics Knowledge of community organization and service system development Problem solving skills and techniques Knowledge and skill in social and health service intervention techniques and methodology.
