Providence is calling a Home Health / Hospice Aide (1.0 FTE, Days) to Providence Medford Medical Center in Medford, OR.

In this position, you will:

  • Provide supportive personal care services to patients of all ages according to the individualized physical needs of the patient in accordance with state and federal regulatory requirements under the supervision of a registered nurse.

  • Perform all duties in a manner which supports team concept of caring and respect, which reflects the philosophy of Providence Home Services, Providence Mission, and Service Excellence Standards of Behavior.

Required qualification for this position include:

  • Current Certified Nursing Assistant (CNA) Oregon certification with 2 years experience in a health care setting providing hands on patient care (exception would be nursing student with current CNA)

  • Current Basic Life Support (BLS) for Healthcare Provider certification

  • Valid driver's license and reliable transportation

  • State Home Health Aide certification Oregon Association for Home Care (OAHC) Home Health Aide (HHA) exam and on-site skills competency to be successfully completed with a score of 80% or better within 2 weeks of hire

  • The HHA training program must meet the state and federal requirements and/or competency exam (also meets state and federal requirements) that include observation of skills. The competency exam, required by federal requirements, is managed by Home Health and Hospice department.

  • Be subjected to complete and pass at hire and annually, a skills observation checklist

  • Must have the abilities to meet the physical and mental demands of this position (see ADA for this position)

  • In compliance with the Providence Health& Services-OR, criminal background check is required for this position, based upon assigned duties and regulation.

Experience * Prior CNA experience required. * Home health, Skilled Nursing Facility, and or private duty care for individuals at end of life, experience preferred * Recent Hospice and end of life care experience required Additional Posting Information * This position requires to take weekend call Working Environment/Physical Requirements This position requires the ability to lift up to 50 pounds on an occasional basis for example to transfer a patient in and out of a bed. The ability to bend, stretch, twist and perform other awkward physical movements is also required. To be placed in these position applicants must pass a physical functional screening.
