Position Summary

The Home Health Clinical Admissions Coordinator is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Clinical Admissions Coordinator is responsible for providing support to the Centralized Intake Team and act as owner of all referrals and admissions. S/he is responsible for monitoring and managing referrals and admissions at the program level. This position is high customer-service oriented and requires clinical knowledge in addition to technical skills. Quality, accuracy, and timelines are critical components to the success of this position.

Position Specific Responsibilities

  • Build and develop relationships with referral sources, creating focus on supporting the local region locations with growth.

  • Responsible for creating an environment conducive for growth and fostered connections with local teams and the Central Intake department.

  • Support the clinical and non-clinical admissions staff by teaching, coaching, and guiding colleagues.

  • Work in the field to support branch admission and visit needs. This may require carrying a patient load expectation.

  • Responsible for taking and obtaining doctor's orders and documentation need for admissions.

  • Verify all referral elements are present for an admission.

  • Maintain consistent knowledge of program referral capacity issues.

  • Participate in regularly scheduled daily stand-up meetings for coordination of services, exchange of information and problem solving, and to receive staff support and education as requested.

  • Develop and maintain a close working relationship with all of the team disciplines.

  • Participate in training courses/sessions as required.

  • Participate in Interdisciplinary Team meetings as needed.

  • Prepare reports as required.

  • Document non-admit referrals.

  • Provide home health information to patients/families as needed.

  • Communicate to staff any referral, admission, and resumption of care.

  • Work closely with PAs for data integrity.

  • Ensure information regarding referral source is accurate.

  • Capture required information on intake form.

  • Perform other duties as assigned.

Education and/or Experience

  • Registered Nurse with a current, unrestricted nursing license in state of employment.

  • Two years of experience as a RN in a clinical care setting, home health preferred.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.

  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications.

  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.

Certifications, Licenses and Registrations

  • Registered Nursing License in state of employment required.

  • Current CPR certification.
