Requirements

To be considered for this position, you must meet the following requirements:

• Hold a valid and unrestricted license as a Registered Nurse (RN)
• Have a minimum of 2 years of experience as a Case Manager in a home health or similar setting
• Possess strong assessment and care coordination skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage multiple cases
• Proficient in using electronic medical record systems
• Have a reliable means of transportation
• Be willing to travel within the designated service area in Cleveland and surrounding areas
