JOB SUMMARY The#Home Health#Aide#is responsible for providing personal care and routine exercise regimes to Home Care and Hospice patients. Responsible for providing homemaking services as assigned. Responsible for timely and accurate documentation of patient intervention on designated Home Health Aide documentation forms. Attends and actively participates in regularly scheduled meetings and in-services. MINIMUM QUALIFICATIONS Education:#See Licensure/Certification Registration requirements. Experience: N/A Licensure/Certification/Registration: Registered on the Minnesota registry upon hire into position. Registered on the Wisconsin Nurse Aide Registry#within 60 (sixty) days of hire into position.
