Summary

We are hiring for a home health aide- this is a PRN position!

The Home Health Aide is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional well being, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures.

Responsibilities

  • Reports any unusual circumstance in the patient's condition or home environment to the primary RN/Patient Care Manager.

  • Follows current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs) to include mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting.

  • As assigned, performs incidental household services essential to the patient's health care at home that are necessary to prevent or postpone institutionalization.

  • Completes a clinical note for each visit within required timeframes, which must be incorporated into the patient's record.

  • Participates in the in-services program. Completes at least six hours of in-services every six months.

  • Participates in daily report. Adheres to and supports care management model within the agency.

  • Participates in staff conference regarding patient care.

  • Participates in QA/QI plan or process.

  • Provides care according to the aide plan of care|assignment sheet and in accordance to State Practice Act or regulations.

  • Records and notifies RN|Patient Care Manager of any variations from the established parameters or changes in the patients condition.

Education and Experience

License Requirements

  • Successful completion of a competency evaluation.

  • Current CPR certification required.

  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Additional State Specific Requirements

  • AL, AZ, GA, KY, MA, MI, MN, MO, NM, OH, SC, WV. No additional requirements.

  • AR: Completion of 75-hour aide training course meeting requirements of AR regulations.

  • CA: Must be certified by the state of California as a home health aide.

  • CO: Must be certified by the Colorado Department of Regulatory Agencies within four (4) months ofstarting employment. Successful completion of a competency evaluation.

  • CT: Must have completed a training program approved by the state of Connecticut of not less than 75 hours.

  • D.C.: Certified by the Board of Nursing

  • DE: Aides who experience a break in service for greater than two (2) calendar years will be required to repeat the minimum 75 hour training requirement or successfully demonstrate competency in each of the required training areas.

  • FL: Successful completion of at least forty hours of training in areas required by Agency for Health Care Administration or successfully passing the home health aide competency test (Form AHCA 3110-1007 created by AHCA).

  • ID: Must be Certified Nursing Assistant and listed on Idaho CNA registry.

  • IL: Must be licensed, registered or certified as a Home Health Aide or approved on the Department's Health Care Worker Registry or completion of training program approved under Long Term Care Assistants and Aides Training Programs Code orequivalency established by 1 of the following: evidence of current registration from another state, evidence of completion of a nursing arts course, successful completion of US military training program, or successful completion of a nursing program in a foreign country.

  • IN: In good standing on the state aide registry.

  • LA: Current CNA certification or successful completion of a training program.

  • MD: Must be certified by Maryland State Board of Nursing (CNA).

  • MO: Certified Nursing Assistant (CNA) preferred. If not a CNA must complete the Missouri WrittenAide Competency Exam and Basic SkillsTest. MS: Successful completion of a competency evaluation and one of the following: 1) previous work experience as a nurse's aide or home health aide 2) completion of approved home health aide course.

  • NC: Must be listed on the North Carolina nurse aide registry.

  • NE: Meet one of the following qualifications and provide proof of meeting the qualifications to the home health agency:

  • a. Has successfully completed a 75-hour home health aide training course which meets the standards described in Neb.Rev. Stat. § 71-6608.01;

  • b. Is a graduate of a practical or professional school of nursing;

  • c. Has been employed by a licensed home health agency as a home health aide II prior to September 6, 1991;

  • d. Has successfully completed a course in a practical or professional school of nursing which included practical clinical experience in fundamental nursing skills and has completed a competency evaluation as described in Neb. Rev. Stat. § 71-6608.02;

  • e. Has successfully completed a 75-hour basic course of training approved by the Department for nursing assistants as required by Neb. Rev. Stat. § 71-6039 and has completed a competency evaluation as described in Neb. Rev. Stat. § 71-6608.02;

  • f. Has been employed by a licensed home health agency as a home health aide I prior to September 6, 1991 and has completed a competency evaluation as described in Neb. Rev. Stat. § 71-6608.02; or

  • NH: Must be licensed as a nursing assistant.

  • NJ: Must be certified as a nurse aide by the Board of Nursing.

  • NV: Must be certified as a nursing assistant from the state Board of Nursing.

  • NY: Successful completion of a basic training program in home health aide services or an equivalentexam approved by the Department with written evidence of completion.

  • OK: Must be certified by Oklahoma State Department and placed on Home Health Aide Registry.

  • OR: Must be certified by Oregon State Board of Nursing (CNA) and inclusion on the Oregon State Boardof Nursing Aide Registry.

  • PA: Required to have completed a minimum of 60 hours of classroom instruction prior to or during the first 3 months of employment.

  • RI: Must hold a certificate of registration as a nursing assistant with the Rhode Island Department of Health. Successful completion of a competency evaluation.

  • TN: Successful completion of a total of seventy-five (75) hours of training which include sixteen (16) hours of clinical training prior to or during the first three (3) months of employment and who is qualified to provide basic services, including simple procedures as extension of therapy services, personal care regarding nutritional needs, ambulation and exercise, and household services essential to health care at home.

  • TX: Must have a minimum of one-year full-time experience in direct client care in hospital or nursing facility, OR one year full- time experience within last 5 years in direct client care in agency setting, OR documented completion of training and competency evaluation program with home health experience within the last 24 months, OR documented completion of competency evaluation program with home health experience within the last 24 months, OR nursing student with

documentation from director of programs or the dean of a school of nursing that states that the individual is a nursing student who has demonstrated competency in basic nursing skills aligned with the school's curriculum, OR be on Texas Department of Human Services (DHS's) Nurse Aide Registry with no findings of client abuse or neglect or misappropriation of client property. Must NOT be listed on the Employee Misconduct Registry, been revoked on the Nurse Aide Registry, or had a criminal conviction as listed in Health and Safety Code Chapter 250.

  • VA: Successful completion of a competency evaluation in-service training.
  • WA: Must be certified by the Washington Department of Health.
  • WI: Must be listed on the Wisconsin Home Health Aide Registry.

Skill Requirements

  • Must be mature and able to deal effectively with the demands of a home health aide position. Able to read, write, and carry out directions promptly and accurately.
