HOME HEALTH AIDE (CNA) Pick your schedule and service area. We are growing and seeking CNA's that live or want to work throughout Hall, Dawson, Lumpkin, Habersham, and White County. Home health experience is not required. You bring the desire to work in home health and we will supply the rest.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan

• Complete all documentation and records in a timely and accurate manner

• Follow established policies and procedures as related to the scope of practice

• Maintain continuity of patient care in a home care setting

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Active, current, unrestricted Certified Nurse Assistant (CNA) certification in the state of practice
