SUMMARY OF JOB RESPONSIBILITIES:
The Home Health Aide is an individual who provides basic care to home care patients of all ages. The Home Health Aide supports the plan of care developed for the patient.
The Home Health Aide is supervised by and directly accountable to the Home Care Nurse who is accountable to the Team Manager.
The Home Health Aide witnesses the agency's philosophy by respecting the uniqueness and dignity of each person and by implementing the values of justice and compassion in providing services and in all professional interactions. The Home Health Aide promotes positive guest relations, demonstrates the agency's core values, and exemplifies the agency's philosophy in daily work activities; promotes and maintains effective relationships and communication in all professional endeavors.
Completion of training course for Home Health Aide. Certification as Home Health Aide. CPR certification.
Orientation as Home Health Aide
KNOWLEDGE AND SKILLS:
Knowledge and skill required to perform the job responsibilities for both technical and dimensional content.
Assists the registered nurse and practical nurse by giving patient care in simple nursing situations; must accurately document procedures performed; must accurately complete assignments and follow directions; must report any unusual observations and patient care needs to the Home Care Nurse.
The Home Care Aide is supervised by Home Care Nurses. The Home Care Nurses supervise patient satisfaction; interpersonal relationships; adherence to policy and procedure; adherence to the plan of care and role as Home Care Team member.
PROBLEM SOLVING/DECISION MAKING:
The degree to which judgment and analysis must be exercised in planning, investigating issues, and evaluating alternative solutions to the challenges posed by the job. Deals with the nature of the decisions, the variety of problems, and how decisions are made in context of the typical requirements of the job.
The Home Health Aide is responsible to perform care and documentation with accuracy. The Home Health Aide realizes that error and incompetence may have serious consequences for a patient.
Regularly exposed to the risk of bloodborne diseases. Contact with patients under wide variety of circumstances. Exposed to unpleasant elements (accidents, injuries and illness). Subject to varying and unpredictable situations. Handles emergency or crisis situations. May perform emergency care. Occasionally subjected to irregular hours. Travel required. Will make home visits.

The Home Health Aide must have the ability to understand written or verbal instructions and be able to communicate necessary information to Home Care Nurses, patients, families, caregivers, physicians and all other individuals involved in patient care. The Home Health Aide promotes positive guest relations, exemplifies the agency's philosophy and demonstrates the agency's core values of compassion, respect, excellence, interdependence and stewardship.
Responsible for proper use of supplies and equipment; responsible for completion of accurate records.
The nature and frequency of interpersonal human relations skills required by the job. the extent to which the job requires cooperation and tact in meeting or influencing others by phone, correspondence, or personal contact.
The Home Health Aide is responsible to keep the patient's medical record and information relating to patients, physicians and employees confidential. Understands that improper release of information may be a direct violation of patients' rights.
PRINCIPAL JOB ACCOUNTABILITIES:
Participates in growth and development for self, patients, families, and community.
1.Participates in at least 12 hours/year of position related educational programs to address personal, patient, family and community educational needs.
2.Documents educational programs in continuing education file.
3.Participates in orientation of newly hired HHA's
Participates in the quality improvement process.
1.Complies with agency policies and procedures.
2.Provides services as outlined in patient care plan.
3.Completes and submits documentation according to policy.
4.Patients' overall appearance reflects good personal hygiene.
5.Patients' environment is neat and orderly.
6.Accurately reports observations of patient condition to RN in a timely manner.
7.Interacts with patients, families and other care givers in an emotionally supportive way.
8.Promptly responds to patient requests.
9.Demonstrates compliance with federal, state and local rules and regulations, along with the Mercy Health Corporate Responsibility Program.
Promotes effective written and verbal communication.
1.Supports and promotes a positive work environment.
2.Participates in at lest 75% of home care monthly staff meetings.
3.Participates in appropriate committee/task force meetings.
4.Demonstrates skill in conflict resolution.
5.Maintains proper lines of authority and responsibility.
6.Addresses and reports patient concerns in a timely manner according to policy.
7.Communicates appropriately with RN regarding patient care and concerns.
8.Maintains availability at all times during work hours.
9.Documents on daily visit log accurately and according to policy.
Utilizes resources in a manner consistent with the Mercy Health Home Care strategic plan.
1.Demonstrates appropriate and cost effective use of supplies and equipment.
2.Assumes on-call responsibility as required by agency.
3.Fulfills additional performance responsibilities as assigned and necessary.
