Family Hospice & Palliative Care OP is seeking a Full-Time Home Health Aide who will provide designated care under the direction of the primary service provider in the home. Responsibilities:

  • Communicate appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.

  • Fulfill agency responsibilities by completing all required forms accurately and in a timely manner.

  • Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

  • Participate in all required agency meetings.

  • Provide safe and direct personal care to patients in the home setting under direct supervision of appropriate professional by performing a variety of assignments as defined in the Home Care Aide Competency Evaluation Program.

Qualifications

  • High school diploma or equivalent.

  • Nursing Assistant Program that meets requirements of 484.36 (b) or (e).

  • 1 year of relevant experience.

  • Must have successfully completed a state established or other training program that meets the requirement of 484.36(a) and a competency evaluation program or state licensure program that meets the requirements of 484.36 (b) or (e) or a competency evaluation program or state licensure program that meets the requirements of 484.36 (a) or (b)of CMS Conditions of Participation.

Licensure, Certifications, and Clearances:

CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance

  • Act 34 Criminal Clearance

  • Automotive Insurance

  • Cardio Pulmonary Resuscitation

  • Driver's License

  • Older Adult Protective Services Act

Collaborates with the Director of Health Care Services (DHCS) to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes in accordance with physician orders, applicable law and regulations, accepted standards of clinical practice, payor requirements and our clients policies and procedures. <br />  <br /> <strong>Essential Functions:  </strong> <ul> <li style="padding: 0; margin: 0;" style="padding: 0; margin: 0;">Prefer at least three (3) years of experience as a home care RN and at least two (2) years of supervisory experience.</li> <li style="padding: 0; margin: 0;" style="padding: 0; margin: 0;">Able to provide proof of valid driver's license, in accordance with office policy.</li> <li style="padding: 0; margin: 0;" style="padding: 0; margin: 0;">Able to provide proof of valid auto liability insurance in accordance with office policy.</li> </ul>  <strong>Knowledge, Skills & Abilities Required:  </strong> <ul> <li style="padding: 0;
