Communicate appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
Fulfill agency responsibilities by completing all required forms accurately and in a timely manner.
Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
Participate in all required agency meetings.
Provide safe and direct personal care to patients in the home setting under direct supervision of appropriate professional by performing a variety of assignments as defined in the Home Care Aide Competency Evaluation Program.
High school diploma or equivalent.
Nursing Assistant Program that meets requirements of 484.36 (b) or (e).
1 year of relevant experience.
Must have successfully completed a state established or other training program that meets the requirement of 484.36(a) and a competency evaluation program or state licensure program that meets the requirements of 484.36 (b) or (e) or a competency evaluation program or state licensure program that meets the requirements of 484.36 (a) or (b)of CMS Conditions of Participation.
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
Act 33 Child Clearance
Act 34 Criminal Clearance
Cardio Pulmonary Resuscitation
Older Adult Protective Services Act
