Provides paraprofessional services to the organization's home care clients as developed by the primary nurse and as prescribed by they physician and in compliance with the state's Nurse Practice Act, and any other regulations defining home health aide / hospice aide services.
Follows care plan developed by RN.
Reports client changes in condition to RN, as described on care plan.
Assists clients with personal care activities including bathing, mouth care,
denture care, skin care, grooming, bathroom privileges, transferring, ambulation,
and dressing and undressing activities.
. Participates in multi-disciplinary case conferences as appropriate.

Submits documentation in a timely manner.
Complies with agency policies and procedures.
Works with scheduling to assure accurate and up to date schedule.
Immediately reports any changes in client's condition or incidents to RN
relationship with clients/families.
