Provides quality care to resident's requiring personal care and home support services to restore and maintain the resident's physical and emotional well-being, while maintaining a safe and clean environment. Under the supervision of the Wellness Director, assists with the physical and personal care of resident's to restore the activities of daily living including nonhealth services to maintain a safe and clean environment, planning and preparing meals, and maintaining current and accurate resident records. The below responsibilities are to be performed on an individual basis only as prescribed in a plan of care ordered by physician.

Essential Responsibilities/Accountabilities:

  • Observes and records resident's care, symptoms, emotional and physical status (vital signs, intake and output).

  • Reports pertinent observations and significant changes in resident's condition to the assigned Wellness Director/Case Manager or designee

  • Accompanies resident outside the home when authorized by supervisor.

  • Assists with medications which are ordinarily self-administered and which a physician has prescribed.

  • Participates in supportive activities to ensure maximum resident independence within the home setting.

  • Consistently demonstrates high standards of integrity by supporting the Community mission and values and adhering to the Corporate Code of Conduct.

  • Maintains high regard for resident and family privacy in accordance with the company privacy policies and HIPAA.

  • Completes all required paperwork in a timely and accurate fashion according to established policies.

  • Provides mentoring/support to other Home Health Aides and Resident Aides as requested.

  • Provides hands on experience to new Home Health Aides and Resident Aides.

  • Works with residents and their respective Case Managers who need additional care.

  • Communicates directly with Physician regarding changes to the resident's condition, when authorized by their supervisor.

Job Requirements:

  • High School Diploma or GED required.

  • All home health aides shall have successfully completed a basic training program in home health aide services or pass an equivalent exam approved by the NYS Department of Health and possess written evidence of such completion. In addition, they will participate in 40 hours of community-specific personal care training and 40 hours of medication administration training.

  • Ability to read, write, understand, and carry out directions or instructions and maintains simple records.

  • Must respect resident confidentiality, have an empathetic attitude toward residents and families requiring care and must continue certification requirements as a home health aide.

  • Pre-employment physical examination required to ensure the applicants ability to perform all responsibilities of the job. Also an annual health assessment as required by NYS Health Department will need to be completed.

  • Any and all other requirements as specified in applicable law and regulation.

  • Demonstrates commitment to outstanding customer service both internally and externally.

VIEW JOBS 9/8/2019 12:00:00 AM 2019-12-07T00:00 Position Summary The primary purpose of your job position is to assist in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor or charge nurse, and the Wellness Director. The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility, and the requirements of this state. You will also serve as Resident Aide as needed. Essential Functions * Must possess the ability to make independent decisions * Must possess the ability to deal tactfully and patiently with personnel, residents, family members, visitors, government agencies/personnel, and the general public. * Must immediately report information concerning a resident's condition to the shift supervisor. * Must supervise and assist with the storage, distribution and discontinuation of residents' prescribed medication. * Must comply with Policies and Procedures of the community as outlined in the Procedure Manual. * Must ensure compliance and understanding of all regulations regarding resident rights. * Adhere to confidentiality policy regarding information concerning residents and employees, HIPPA. * Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information. * Comply with the Residents' Rights and Community Policies and Procedures. * Must have compassion for and desire to work with the elderly. * Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. * Must be willing to work a flexible schedule, including weekends as needed for the residents. * Must be a supportive team member, contribute to and be an example of team work and team concept. Skills and Abilities * Must possess, as a minimum, a high school education or its equivalent. * Must be able to read, write, speak, and understand the English language. * Must be able to assist in the evacuation of residents. * Must be computer literate and able to utilize company chosen software. * Perform work tasks within the physical demand requirements of your position including standing, sitting, walking, lifting, carrying, pushing, pulling or otherwise move objects for extended periods of time. Certificates and Licensing * Must be a licensed Certified Medication Aide having successfully completed a state approved training program and any necessary examination(s) in accordance with laws of this State. * Must have CPR certification upon hire or obtained during orientation.
