Home Care through Spectrum Health NRS offers a highly skilled team of specialists working together to help individuals reach their greatest possible independence and the best possibly quality of life. Our professional treatment team takes a multidisciplinary approach to care, developing individual plans for each patient to ensure optimal rehabilitation.
Our home care program is designed to help children and adults with complex medical and rehabilitation needs, such as traumatic brain injuries, stroke, neurological conditions, spinal cord injuries and orthopedic injuries. Care is provided by our highly skilled specialists, up to 24 hours a day, seven days a week. We are here to help patients and their families get back to enjoying life, surrounded by the familiar comforts of their own home.
Under the direct supervision of the Nursing RN case manager, and in collaboration with the interdisciplinary team, the Home Health Aide (HHA) serves the role in assisting our clients with personal care activities ranging from ADLs bathing, dressing, feeding to client transport and meal prep. Home Health Aides work in assisting with task directly essential to the client's health in accordance with their care plan; observe, record, and report services provided and clients response to care as well as client status to the case manager. Home Health Aides strive in working diligently to help our clients reach their maximum potential
