Position Summary
This position is for a paraprofessional providing care to patients to improve the quality of the patient's life in the home environment. The HHA performs tasks required to help maintain and restore the physical, social, and emotional health of clients, and this care is under the direct supervision of a registered nurse. This position reports to the Clinical Manager.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: High School diploma or GED preferred. Must also have the willingness and ability to obtain a minimum of 12 hours of education annually.
License/Certifications: State of Maine certification as a Certified Nursing Assistant. Current, valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy. Current certification in Basic Life Support for Healthcare providers.
Experience: One year experience working as a CNA or HHA preferred.
In this role you may:
Works within the scope of practice for Home Health Aides, under the supervision of the registered nurse (or therapist), providing quality care while following the Home Health Aide Plan of Care.
Makes and adjusts daily client schedules to meet client and agency needs, keeping Scheduling and their supervisor aware of their whereabouts and availability to accept additional work.
Calls ahead to verify patient is available and wiling to accept the home care visit.
Recognizes and addresses family concerns related to the client's health and functional problems.
Talks with their supervisor and the patient's primary nurse as needed.
Provides professional care and great customer service to al patients.
Obtains a minimum of 12 hours of education appropriate to their role as a HHA.
Completes clear visit documentation and submits according to the agency policy.
Maintains confidentiality in both oral and written communication to protect client's rights.
Notifies supervisor immediately when any issue arises that may present a risk to the Agency (such as customer complaints, patient care issues, safety concerns, etc.)
Assists with Telehealth installations as requested
