Job Description
JOB SUMMARY:
The Hospice Home Health Aide is a certified nursing assistant. Under the guidance and supervision of a registered nurse or therapist, provides personal care and related services for the hospice patient in order to augment care provided by the family and other caregivers and to maintain a clean and safe environment. Hours may vary depending upon census and program need.
EDUCATIONAL REQUIREMENTS
H.S. Diploma/GED
CERTIFICATION & LICENSURE REQUIREMENTS
Driver's License upon hire and Proof of Auto Insurance upon hire.
EXPERIENCE REQUIREMENTS
One (1) Healthcare/Medical
ADDITIONAL REQUIREMENTS
Certified Nursing Assistant (CNA) Illinois
Proof of successful completion of 120 hours of state approved Nurse Aide training with employment within 24 months of course completion. Satisfactory completion of written exams and return demonstration of identified skills.
SKILLS AND KNOWLEDGE
Interpersonal skills necessary for establishing and maintaining good working relationships with the patient, family, and interdisciplinary team. Drives reliable transportation and maintains state required insurance and Illinois driver's license in good standing.
ESSENTIAL FUNCTIONS:
Completes documentation as per policy and procedure in order to maintain a thorough and accurate clinical record for each patient.
Reports any changes in patient status to the licensed staff.
Practices in a manner sensitive to the needs of patients and families. Provides care according to plan of care and orders.
Assesses, identifies and reports safety issues for patient and staff.
Provides sufficient support for use of off-formulary items and gets appropriate authorization from supervisor/manager before calling for order.
Orders majority of items from established formulary. Uses correct item number and sufficient information to eliminate repeat contacts before order can be placed. Documents identifiable supply information in electronic chart.
Captures all legitimate revenue; conservative in expense and resource use yet provides appropriate service and supplies to patients.
Assists the patient in ambulation, exercise, and use of walker, cane or wheelchair as instructed. Assists with activity prescribed by physical or occupational therapist as instructed.
Perform or assist a patient with the personal care and treatments needed to provide good personal hygiene and a healthy, safe environment as directed by the plan of care and by a registered nurse or therapist. Performs homemaking tasks as assigned.
Assists with treatments as ordered by the physician and instructed and supervised by the nurse, such as change of a colostomy bag, shampoo, simple wound care, reinforcement of a dressing, simple urine tests for sugar, acetone, and albumin, and measuring intake and output.
Encourages patient's mental alertness through involvement in activities of interest.
Gives simple emotional and psychological support to the patient and other members of the household or facility.
Encourages the patient to become as independent as possible according to the plan of care.
Assists patients with bathing and personal care as directed by the plan of care.
Monitors vital signs as directed by the plan of care.
Monitors skin integrity and provide skin care as directed by the plan of care.
Provides hair care including shampooing as directed in plan of care.
Attends IDT meetings to provide patient care updates (as schedule allows)
