Colorado Visiting Nurse Association has an immediate opening for a Home Health Admissions & Scheduling Coordinator.
Location: Arvada, CO
Status: Full-time
Schedule: Mon-Fri, 8-5 with rotating weekends
Pay Range: $20.14 - $25.08 per hour
Supplemental Pay: Based on position, schedule and/or availability: Overtime
Mileage and Expense Reimbursement: Sixty-two and a half cents per mile - one of the highest in the industry!

REWARDING WORK YOU WILL DO:
Provides excellent service to internal and external customers regarding Home Health services. Coordinates the intake of referrals, admission of patients and scheduling of clinicians according to admission criteria and appropriateness. Collaborates with team members as to the appropriate programs and requests assessment visits. Schedules clinicians to ensure regulatory and patient safety needs are met.
Answers incoming calls and provides information regarding home health care programs and admissions criteria to external and internal customers.
Works closely with Business Development and/or Clinical team/staff to ensure timely patient care.
Receives and documents complete referral information, schedules assessment using the EMR, and coordinates the admission process.
Manages multiple referral portals simultaneously and responds to referral partner in a timely manner, verifying staffing availability, service area and insurance within network. Collaborates with manager for clinical appropriateness.
Verifies that each day's visits, both new admissions and re-visits, are properly scheduled for the correct day and time and identifies trends in non-compliance.
Performs several scheduling functions including daily/weekly scheduling for assigned teams.
Uses effective organization, time-management, and prioritization skills. Ability to handle multiple tasks from multiple sources in a busy setting with frequent interruptions.
Facilitates flow of patient information and communication. Utilizes sound judgment when handling telephone calls and triaging to appropriate clinician.
Accurately assign new referrals to caregivers on a timely basis. Assigns based on patient's needs, caregiver's skills, physical locale, and caregiver's caseload. Assures visits are properly scheduled out for the correct day. Notifies Manager promptly of staffing problems and or inability to meet productivity expectations for clinicians.
Communicates appropriately with caregivers, patients, physicians, and other contacts and accurately. Relays messages as needed.
May need to assist with hospice referrals and admissions as needed.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Minimum Education: High school diploma. Degree from an accredited university in medical, business, or human relations preferred.
Minimum Experience: One (1) year clerical/customer service experience/data entry (home health preferred). Experience in extrapolating referrals from portals preferred.

Colorado Visiting Nurse Association is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado's Front Range and the Region's Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip Colorado Visiting Nurse Association to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
