JOB SUMMARY This position is responsible for maximizing patient and family engagement, communication and coordination of care among all care team members in order to provide timely, safe and high quality care during transitions related to homecare and hospice services. This individual will have knowledge of admission criteria and requirements of the regulatory bodies governing home care and hospice. Focus will be on educating hospital patients and their families about homecare, hospice and palliative care services, provide orientation for new hospital staff members on home care, hospice and palliative care services, speak at staff meetings about home care, hospice and palliative care services; advise hospital staff about Medicare, Medicaid and private insurance reimbursements for home care and hospice programs. Excellent clinical knowledge is essential to this position. This individual will serve as a resource to the health care team on home health issues. Effectively interacts with patients, families, case managers and physicians in communicating services provided to meet the individual client#s needs in a timely, professional manner. This position will need to meet a minimum productivity standard as determined by Homecare and Hospice administration (i.e. administrative director, director of homecare and hospice or their designee). This position requires full understanding and active participation in fulfilling the Mission of Blessing Corporate Services. ICARE standards must be demonstrated. Adherence to Code of Conduct and other policies is required. JOB QUALIFICATIONS Education/Training/Experience: REQUIRED: Graduate of an RN School of Nursing Excellent written and verbal communication skills Must have a minimum of 2 years RN clinical experience, preferably home health and/or hospice Registered Nurse licensed in Illinois and Missouri Candidate must demonstrate comprehensive understanding of home health services. Self-confident individual with outgoing personality who functions effectively in an autonomous environment. Previous sales and marketing experience highly desirable. Self-directed individual that is detail focused and possesses the discipline to establish positive relationships with targeted facilities and/or physician practices. Basic computer skills necessary. Demonstrated ability to work with all individuals on health care team. Capable of working independently as well as part of the team. Capable of adapting to changing environment. Ability to understand and effectively communicate in both written and oral form and handle sensitive telephone and face-to-face inquires. PREFERRED: Preferred BSN License/Certifications/Special

JOB QUALIFICATIONS

Education/Training/Experience:

  • REQUIRED:

  • Graduate of an RN School of Nursing

RNs in this role work autonomously alongside the hospital staff to assess, troubleshoot, and make sound decisions for the critically ill patients. Acute RNs may deliver nephrology care across multiple renal therapies: peritoneal, continuous renal replacement and apheresis. * Autonomy in a hospital setting. Deliver dialysis to patients at a particular hospital or group of hospitals and become an integral part of their team and ours. As a teammate in an acute setting you work on your own almost all of the time. Each day you assess, troubleshoot, and make sound decisions for critical patients. Our Hospital Services group performs 1.1M treatments annually. * Dynamic environment. No two days are ever the same for an acute dialysis nurse. You'll work within a variety of settings (ICU, CCU, ER and more), and you'll be able to keep learning new nursing skills on top of dialysis. * Schedule: Provide emergent, lifesaving care during on call shifts.
