Primary Care RN Home Care
JOB SUMMARY
The RN case manager is responsible for providing skilled nursing care and the oversight of care provided by home health/hospice aide. The RN is responsible for the coordination of all patient care services. The Primary Care RN supports the company philosophy, goals and culture.
Qualification Requirements
Graduate of an accredited RN program. Connecticut State Licensed Registered Nurse with a minimum of one year of clinical experience. Previous home care experience desirable. Must possess basic computer skills, and excellent documentation and customer service skills

Essential Job Responsibilities

Provides skilled nursing care including administration of medications and treatments as prescribed in physicians plan of care for patients in their home setting.
Coordinates plan of care with all disciplines associated with patients needs, including family and primary care givers.
Observes, evaluates and reports on the patients physical and emotional status to nursing supervisor and the attending physician whenever change occurs or at least every 60 days.
Attends all required staff meetings to enhance professional development
Maintains and processes all documentation in compliance with agency standards
Will utilize computer based documentation system and submit all required clinical documentation, time records and other required information in required time frame.
Instructs, orients and supervises other clinical personnel as necessary
Develops plan of care with instruction for home health aide services.
Provides input to Home Health Aide Supervisor as needed for purpose of home health/hospice aide evaluation.
May act as a liaison with institutions to promote continuity of care as the patient moves from level of care to another.
May participate in community activities as a representative of the agency.
Participates in weekend rotation and on call rotation as needed.
May perform other duties as assigned.

Theyare responsible for providing support and performing duties in a manner thatensures effective and efficient customer service and patient care. This position is ideal for someone with a bachelor's level social serviceeducation and skilled nursing facility professional background that ispassionate about transforming healthcare and helping patients. Key Areas of Responsibilities: * Performs care coordination services for ICP attributed patients admitted to ICP PPN SNFs * Participates in care planning, family, and discharge planning meetings throughout patient's SNF stay * Partners with ICP PPN SNF discharge planners to facilitate clinically appropriate LOS for ICP attributed patients * Collaborates with ICP Community Care Management team and other HHC system partners upon patient's discharge from ICP PPN SNF * Identifies resources within HHC system and larger community to address patient care needs * Provide education for patients, families, community, and other professionals regarding disease prevention, impact of illness and advocacy for benefits and health maintenance Reports To: Manager, Care Management Nature of Supervision: External: External customers of Integrated CarePartners, system based clinical positions requiring support of ICP Clinicalteam. Education: Bachelor'sdegree in Social Work/Human Services/Social Sciences is required. License: Mental Health First Aid Certified Language Skills: * Able to read, speak, write English Language Skills * Ability to communicate in other languages highly desirable Knowledge, Skills andAbility Requirements: * At least two years of experience working in a skilled nursing facility * A passion for helping vulnerable populations * Knowledge of medical terminology * Willingness to develop new skills and ability to receive feedback * Positive, customer-focused approach * Proven ability to work effectively independently as well as in a team environment * Must be comfortable and flexible with ambiguity and change processes * Excellent written and verbal communication skills * Strong organization skills and attention to detail * Establishes and maintains effective work relationships. * Displays courtesy and helpfulness when interacting with all internal and external customers * Proactively takes action to achieve goals beyond what is required or assigned * Adheres to HIPPA requirements * Adheres to Hartford Healthcare policies and standards of service, as well as those of practices being supported. * Physical abilities to include sitting and standing for prolonged periods * Must attend and pass examination for Mental Health First Aid Certification within 60 days of hire We take great care of careers.
