Position Details
The Home Care Manager is responsible for planning, organizing, and managing a team of Home Care professionals and ensuring quality and excellence in the delivery of services to clients. As the Home Care Manager, you will develop and monitor plans of care for the home and community-based clients and ensure consistent provision of quality services in keeping with client goals and objectives, Family First Mission, policies, and program requirements, as well as requirements of the Home Care industry and other regulatory agencies. This position is also responsible for managing and developing home care team members.
Job Duties
Works in a collaborative / team-based manner that supports and promotes Family First vision, mission, and values.
Presents performance reports and metrics to the Director of Home Care and other leaders.
Adheres to and demonstrates Family First missions, vision, and values.
Establishes and maintains an environment of psychological safety that promotes teamwork and cross-functional collaboration across all departments.
Completes the required Quarterly Reviews and Annual Assessments.
Manages client caseload for assigned clients in assigned territory.
Completes care plan changes as needed and identifies options for service delivery, including the type of service(s), program eligibility, and payment source(s) in keeping with Family First philosophy and program requirements.
Appropriately identifies cases for case conferencing and confers with the Home Care Coordinator as needed.
Record all client-related activity in the case record and ensure Home Care Coordinator and other staff members follow appropriate documentation protocols.
Maintain up-to-date client files and plans of care.
Uses the Well Sky software program to enter all client data into the system.
Assures continued identification of new resources to assist clients and their caregivers.
Handles client or provider concerns or requests regarding service, billing, or other needs, assuring excellent customer service.
Keep abreast of all home and community care rules and regulations; ensures that services and activities adhere to program requirements.
Stays up to date on the latest clinical compliance requirements and adherence to company standards.
Develops and maintains the Home Care Department's direction and builds a team to meet business and operational needs.
Leads a team of Home Care professionals across all Family First training staff, planning, assigning, and directing work, and addressing performance issues.
Build team alignment on divisional and team strategy, goals, and objectives through clear communication, expectations, and team meetings.
Works to achieve established productivity standards in the areas of:
Caseload per caregiver
Billing units per home caregiver
Average cost per home and community care unit for Average care plan costs per caregiver. Average home and community care costs per client.
Identify, track, and evaluate key performance indicators for the Home Care department by regularly reviewing operational procedures and implementing process improvements on an ongoing basis.
Ensures required policies and procedures are adhered to and the required reports are complete and accurate.
Qualifications
Associates degree in healthcare, business, or related field; Bachelor's degree preferred
Current HCA/CNA certification, or willingness to obtain certification within 200 days of employment
3-5 years of experience in Home Care
2 years of progressive management experience leading a team
Subject matter expert regarding principles, procedures, and industry best practices
Strong technical skills with advanced user experience with all Microsoft Office applications
Strong leadership and coaching skills
Strong time management and organizational skills
Delegation skills to ensure direct reports are collaborative, competent, and engaged.
Experience working in a performance driven environment with KPI's
Ability to meet deadlines while managing multiple priorities
Proven ability to work both independently and collaborate with different levels of employees
It may require extended work periods at a desk and computer for up to 8 hours per day
Compensation: Family First is committed to providing market competitive, equitable pay to all employees
$29.00-$35.00 Per hour

Work Location:
Spokane, Washington
Hybrid work opportunities available upon successful completion of training/orientation periods.
Schedule:
FT- 8-hour shift Monday through Friday 8:00AM to 4:30PM
This position may be required to cover on call shifts and provide guidance after hours.
