Home Care LPN

POSITION SUMMARY: Under the direct supervision of an RN, provides practical nursing care for patients following established standards and practices.# Performs treatments and procedures within the scope of the Nurse Practice Act of Illinois.# Updates RN with timely information regarding status of patient in compliance with Home Health Department policy and other regulatory agency requirements.# Participates in continuous quality improvement activities and educational experiences in support of departmental and divisional nursing philosophy and objectives, as well as Health System initiatives.

Compensation is hourly. # EDUCATION/TRAINING: Graduate from an approved licensed practical school of nursing.# At least one year of professional nursing experience.# Community/Home Health experience preferred. LICENSURE/CERTIFICATION:
