Home Care / Home Health Licensed Nurse (LVN) Long Beach, CA (PRN / On-call)

We have an exciting new employment opportunity for a Home Health Licensed Vocational Nurse (LVN / LPN) in the Long Beach, California area. The Home Care LVN is responsible for ensuring high quality care for all clients.

We require a Licensed Practical Nurse / LPN LVN with a minimum of 1 years experience in a home care environment. Strong clinical skills are required. This is a PRN / On-call position.

Home Health LVN / LPN responsibilities:

Scheduling and coordinating care

Performing field visits

Reviewing documentation to ensure regulatory compliance

Assessing the needs of patients & families, providing care per physician order, addressing symptoms, and ensuring comfort

Developing and implementing a care plan to provide restoration of functional capacity, control of symptoms, and relief of suffering while respecting the patients goals and family values

Anticipating disease progression and implications for symptom management

Collaborating and communicating with the Interdisciplinary Team including physician when appropriate

The ideal candidate enjoys working with a diverse team of health care professionals who are committed to providing excellent care, is comfortable with changing demands and priorities, is flexible, has excellent communication skills, and is a good problem-solver.

Home Health Nurse qualifications:

Current nursing license in good standing

Very strong clinical skills

Medicare & OASIS expertise

Prior Home Health / Home Care nursing experience

Experience with MS Office Suite and most EMR programs

Impeccable verbal and written communication skills

The job The Bilingual Home Advantage Care Navigator enhances seniors' ability to manage their health and to live safely and independently in their home by supporting the Home Advantage Occupational Therapist (OT) in developing and implementing home safety plans. You will * Receive completed Occupational Therapist Home Safety Evaluation report and drafts the member's Home Safety Plan that includes the OT recommendations. * Conduct home visit following OT visit in order to finalize the Home Safety Plan with the member. The Safety Plan is designed to help supports member safety, prevent falls and injury. Helps member to understand how to implement the OT recommendations, utilizing coaching/education, member's support system, and community resources. Refers to care management as appropriate. * Provide ongoing telephonic communication as needed and requested by member to support the safety plan, member's understanding and use of benefits, assistance in scheduling appointments, development of a support system, and reinforcement of self-management. * Assist members and caregivers in taking an active role in their health, safety and independence through coaching, education, navigation and referrals to appropriate care and community based resources. * Complete timely and accurate documentation to record home visit, telephonic communication with members, and all other activities. * Utilize department desktop procedures, workflows, job aids and training materials. * Maintain the member's right to privacy and protects SCAN by keeping information confidential. * Adhere to all quality, compliance and regulatory standards to achieve desired outcomes. * Actively support the achievement of SCAN Vision and Goals. Requirements Your qualifications * Associate's Degree in Social Work, Human Services or a related field or equivalent work experience * Strong interpersonal skills, including excellent written and verbal communication skills. * Strong organizational skills. * Ability to multitask * Must be bilingual in English and Spanish: conversation, reading and writing. * Proficient in Microsoft Outlook, Word, Excel and PowerPoint * Valid California Driver's license. Preferred qualifications * Bachelor's Degree * Two (2) to three (3) years of experience working with older adults * Comprehensive knowledge of community and public resources serving older adults What's in it for you?
