UPMC Home Health is hiring a Home Care Aide to join its team and support Mercer and Lawrence Counties!
This is a Part Time position. Typically the schedule will be 2 days one week, 3 days the next. On Call/Weekend/Holiday availability required.

Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.
Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
Proven ability to facilitate change.
Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
Participates in all required agency meetings.
High School Diploma or GED - Required
Must have successfully completed a training and competency evaluation program as specified in 484.80 (Home Health) and/or 418.76 (Hospice) as applicable: Condition of participation: Home Health aide services.
One (1) year experience - Preferred;
Active employment within the past 24 months as a Certified Nurse Aide in good standing.
Licensure, Certifications, and Clearances:
Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. Required CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
Certified Nursing Assistant (CNA)
Act 33 with renewal
