Position Status: Full Time FLSA Status: Non Exempt

POSITION SUMMARY:

The HIV Care Coordination Program (CCR) addresses HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approach to meeting the needs of persons through team-based care management. The program goals include prompt linkage and ongoing retention in HIV care, optimal treatment adherence, viral suppression, and reduced HIV-related mortality. This is achieved through activities such as patient navigation, directly observed therapy, health education, and coordination with the medical team; the frequency of support is driven by the intensity of an individual's need. The program is community-based and requires in-person activity. The program is comprised of 4 Navigators, 1 Coordinator, and 1 Data & Quality Specialist with oversight from the Director. The Health Navigator (HN) carries out activities necessary for individuals to reach these goals and moves them towards self-managing their care and treatment. The Navigator is instrumental in conducting the day-to-day services and works closely with the team and Coordinator.

DUTIES & RESPONSIBILITIES:

  • Accompany clients to medical appointments and community agencies for support and advocacy

  • Coordinate logistics for care and treatment needs including appointment reminders, transportation, and childcare arrangements

  • Provide individual health education to clients regarding curriculum topics as they relate to the patient's life

  • Assess treatment adherence and addresses barriers as they arise

  • Reassess client needs including the Self-Management Assessments and program eligibility

  • Participate in the development of the care planning activities

  • Conduct modified Directly Observed Therapy (mDOT) services

  • Coordinate with Service Providers and provides critical feedback to members of the team

  • Actively participate in case conferences, clinical supervision, and individual supervision

  • Meet training requirements as indicated by the agency and funder, including the 6-day Core training.

  • Adhere to standards for documentation requirements, and data submission and maintains client charts

  • Perform all functions in alignment with CHASI's Mission, Vision, and Core Values

  • Other duties as assigned

Requirements

QUALIFICATIONS:

  • High school or equivalent with related experience and strong socio-cultural identification with the target population or certification as a peer worker with at least 12 months experience in patient navigation REQUIRED; Some college, Associates, or bachelor's preferred.

  • At least 1 year of experience with case management and/or patient navigation REQUIRED.

  • Bilingual (English/Spanish) preferred.

COMPETENCIES:

  • Computer literacy in Microsoft products: TEAMS, Outlook, etc., and ability to navigate the internet.

  • Knowledge, understanding, and ability to communicate HIV progression, treatments, and adherence strategies.

  • Knowledge and understanding of care management principles and documentation.

  • Can actively listen to feedback and apply that feedback accordingly.

  • Compose documentation, emails, and other communications that are clear, accurate, and well-organized.

  • Understand and convey information and instructions so that the message accomplishes its intended purpose clearly with colleagues, clients, and external parties.

  • Work harmoniously with others to accomplish objectives, gaining the cooperation and respect of others in the process. Relates effectively with staff, clients, supervisors, and senior management.

  • Show up professionally each day within the context of expectations of their work environment.

  • Work independently following receipt of instructions on a task.
