Location: 56 Bay Street, Staten Island, NY 10301

Position Status: Full Time

FLSA Status: Non-exempt

Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities - people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY:

The Health Care Coordination Department provides access to health care services for persons living with chronic illnesses including but not limited to HIV/ AIDS, mental health, and substance use disorders. The Engagement and Supportive Services (ESS) program's Case Manager is the team leader ensuring overall health and wellness for persons living with HIV and achieving optimal treatment adherence with the goals of viral suppression, retention in care, and reduced HIV-related mortality. Program clients include persons who are not Medicaid eligible, immigrants, persons newly diagnosed, and those who have fallen out of, or are sporadically engaged in HIV care and treatment. The Case Manager refers, links, and coordinates services including health care, legal, immigration, housing, benefit programs, mental health treatment, and other supportive services. Services are provided in Spanish/English by the bilingual Spanish speaking Case Manager.

DUTIES & RESPONSIBILITIES:

  • Identify eligible clients and program referrals by sustaining relationships with community-based providers and HIV Testing programs.

  • Complete program documentation such as Intake, Assessment and Reassessment by identifying client needs and eligibility for the program.

  • Assesses care and treatment adherence and addresses barriers as they arise.

  • Develop and update service plan goals using a client-centered approach.

  • Conduct case conferences and coordinate services and referrals with providers that facilitate a client's engagement and retention in care.

  • Negotiate and advocate on behalf of clients for services that support self-sufficiency and self-management.

  • Address and remove barriers to enable client access to all necessary components of health care, including mental health and substance abuse services.

  • Accompany clients to medical appointments and community agencies for support and advocacy.

  • Engage clients who are not in care or sporadically in care by support clients in activities that improve their health outcomes resulting in sustained viral load suppression.

  • Coordinate logistics for care and treatment needs including appointment reminders, transportation, and childcare arrangements.

  • Collaborate with the Health Educator to identify needed and appropriate health education services for clients and program needs.

  • Provide health education to individuals as needed.

  • Coordinate with the Peer Navigator to engage clients in care, treatment and support services and to assign client tasks.

  • Complete training requirements and participate in learning opportunities.

  • Adhere to standards for documentation requirements, data submission, and client charts.

  • Perform all functions in alignment with CHASI's Mission, Vision, and Core Values

  • Other duties as assigned.

Requirements

QUALIFICATIONS:

  • Two (2) years of experience working in the field of HIV/AIDS, behavioral health, substance abuse, health education, or other chronic illnesses REQUIRED

  • Fluency in Spanish & English REQUIRED

  • Familiarity with regional HIV primary care, mental health, substance abuse and other services and resources

  • Knowledge, understanding and ability to communicate HIV disease progression, treatments, and adherence strategies

  • Knowledge and understanding of care management principles, documentation, referral processes, and follow up

  • Possess an understanding of community level work and the importance of collaborating and coordinating with other organizations

  • Associates or Bachelor's degree in a related field preferred

  • Effective communication skills, including ability to follow instructions, listen, communicate effectively in writing and verbally; able to organize thoughts logically and explain clearly

  • Culturally responsive to individuals living with HIV/AIDS and those at-risk for becoming infected with HIV, especially in immigrant populations
