POSITION PURPOSE

This position has primary responsibility for case management of patients and their families, coordinating and monitoring the quality of care and treatment planning process for the duration of their stay in the Hospital.

ESSENTIAL FUNCTIONS AND BASIC DUTIES

Case Management: Facilitates patient care by determining and reviewing ongoing medical criteria standards and benchmarks.

Facilitates and effective transition for patients and their families.
Coordinates patient's transition in accordance with utilization review guidelines and admission criteria, through the appropriate services.
Communicates and consults with clinical and utilization review staff, regarding patient progress and movement through system. Updates patient and family situations daily.
Plans for patient's discharge, making all necessary arrangements, contacts and completes placement prior to discharge.
Meets with families on individual bases to prepare for discharge.
Acts as a liaison between patients/families and other health care providers, agencies and payers.
Responsible for patient/family education as well as staff education.
Provides extensive clinical support to families in crises.
Provides education to patients/families for continuing care options.

Team: Acts as a departmental and hospital resource through informal leadership and technical expertise.

Acts as a mentor to others on team.
Participates in the orientation of new staff members, assists with supervision of students and volunteers as needed
Provides education for hospital staff, including physicians, nurses, discharge planners, social service workers and others.

Development: Maintains current knowledge, certification, and licensure.

Identifies learning needs in self and initiates actions to address these education needs.
Attends continuing education programs in order to maintain certification.
Attends hospital sponsored training programs as required by the department.
Assists in the development of new programs to improve quality of care and/or reduce unnecessary expenses.
Completes all mandatory requirements, certifications, and licensure within allotted timeframe for completion (License, PPD, Competencies, Age Specifics, etc.)

Organizational knowledge: Works effectively within and between departments.

Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate
Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees.
Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit.

QUALIFICATIONS

Education:

Associates degree/ Diploma in Nursing, Bachelor's degree preferred

Certification:

Maryland Nursing License

Required Knowledge:

Ability to read, write, speak, and comprehend English.
Basic Computer skills to perform Data entry
Computerized clinical information systems: Clinicians View, Chart View.
Knowledge of UR/admissions criteria for all levels
HIPAA policies on Confidentiality

Experience Required:

Minimum 3 years experience in discharge planning, utilization review or case management in acute medical inpatient settin

Provides care for the adult and geriatric population under the direction and supervision of an RN. ESSENTIAL FUNCTIONS AND BASIC DUTIES Clinical: Performs basic, routine and specialized tasks in assigned department, under the supervision of a registered nurse. Meets patients needs (to include providing baths, linen changes, bedpans, water pitchers, etc.). Assists with ambulation, positioning, elimination, personal hygiene, activities of daily living, routine comfort measures. Transports patients and transfers patients between stretcher and bed and bed to chair. Provides nourishments as requested and authorized by orders, including distribution and collection of meal trays as needed. Participates in shift report, reports observations, unusual happenings to RN. Maintain the comfort, privacy, and dignity of each patient in the delivery of services to them. Observes, monitors, and reports changes in patient's condition, symptom, reactions or behavior as appropriate. Rounds on all assigned patients based on established standards. Documents vital signs promptly and accurately. Purposefully rounding at a minimum of every two hours on assigned patients Environmental: Maintains a clean, safe, prepared work environment. Prepares patient areas for admissions. Cleans patient care equipment, as needed, or as assigned. Distributes supplies to designated storage areas when they are delivered to unit. Checks par levels of supplies and restocks as needed. Maintains orderliness of utility, storage, nourishment and nursing station areas at the end of each shift. Checks sterile supplies for sterility integrity, where applicable. Administrative: Assists in the timely collection and accurate reporting of patient data. Observes, monitors and records vital signs and nursing care procedures in the appropriate area of the clinical record. Assures that documentation is accurate. Promptly answers call lights and telephones Development: Maintains current knowledge, certification, and licensure.
