Job Description:

• Plans, directs, coordinates, delegates and evaluates the activities of all nursing personnel within assigned area.

• Conducts daily resident rounds to observe and evaluate the physical and emotional status of each patient.

• Reviews resident care plans for appropriate goals, problems, interventions, and revises as needed, based on identified nursing needs.

• Ensures the continuity of resident care by informing supervisors and staff of patient status and related matters through written reports and verbal communication.

• Conducts daily conferences with unit staff to review specific patient care, and to communicate and interpret recent policy and procedure changes.

• Extends courtesy to all unit visitors and assists them to contribute to the emotional needs of all patients.

• Coordinates with appropriate personnel to ensure adequate supplies and equipment are available on assigned unit.

• Provides consultation for unit personnel; resolves immediate problems and communicates action to Unit Supervisor.

• Maintains records, files, and reports as directed.

Shifts Available:

• Day

• 7:30am-3:30pm

• Evening

• 3:30pm-11:30pm

• Overnight

• 11:30pm-7:30am

Qualifications:

• NYS LPN License
• 6+ months of experience in a similar setting
