To be fully engaged in providing Quality/No Harm, Customer Service and Stewardship by maintaining accurate and complete medical records for each patient treated at Health First, assuring that the confidentiality, security, and integrity of the data and information is maintained, and provide complete and timely use of data and information, without compromising security and confidentiality. Responsibilities may vary based on job role within the department.

Primary Accountabilities

Demonstrates competence in the computer applications necessary to carry out work responsibilities.

Proficiently navigates through the computer applications in order to identify patient by name, account number, medical record number or date of service and ability to locate appropriate chart based on that information and verifying accuracy of demographic area.

Proficiently navigates through Microsoft Word and Excel

Responsible for verifying and sending reports and/or previous records to surgery queues or physician offices for scheduled visits 98% of the time.

Preps all document batches for scanning according to departmental policy and procedure at least 98% of the time

Scans batches/documents and writes scanned and indexed batches to the appropriate queue according to departmental policy and procedure at least 98% of the time Indexes documents according to departmental policy and procedure to the correct patient, account and document type at least 98% of the time Performs Adjust Document Index function to include correctly inserting, splitting, moving, and appending pages/documents when and where appropriate at least 98% of the time

All discharged charts are reconciled in the HIM department upon discharge. Checks daily discharge log to ensure receipt of all records and follows up on records not received at least 98% of the time.

Ensures that all patient records are received within 24 hours of discharge, reporting to HIM supervisor or HIM manager when unable to obtain a patient record.

Takes initiative to take on additional assignments or changes in scheduling based on daily observations in a willing manner to include all HIM Technician I responsibilities.

Understands and supports the processing of reports and charts to department turn-around-time goals

Reviews assigned work queue(s) daily and ensures processing of assignments in the queues

Productivity standards are met according to departmental policy and procedure for each job function on a weekly basis at least 98% of the time

Completes flags within 24 hours or sooner as appropriate.

Provide patient information to appropriate requesters in an accurate, timely and efficient manner according to departmental policy and procedure at least 98% of the time.

Understands the confidentiality of patient information and proper dissemination of this information.

Adheres to all Health First policies and procedures in regards to protected health information. Consults with management to clarify any concerns.

Provides front desk coverage to process incoming requests received by phone, fax, and in-person.

Consistently exhibits exceptional customer service skills and professional communication both verbally and in written format.

Scan and index all patient requests on a daily basis before the end of shift.

Always displays professionalism and respect, every person, every time.


  • High School Graduate required.

  • Previous healthcare related experience preferred.

  • Proficiency in Outlook, Word, and Excel programs.

  • Basic knowledge of Joint Commission and HIPAA preferred.

VIEW JOBS 4/21/2019 12:00:00 AM 2019-07-20T00:00 POSITION SUMMARY: In alignment with Health First commitments of Quality / No Harm, Customer Experience, and Stewardship, the Medicare Quality Programs Coordinator-LPN ensures that patients receive recommended preventative screenings and medical assessments. The position contributes to Risk Adjustment activities that support compliance with regulatory and contractual requirements specific to Medicare. PRIMARY ACCOUNTABILITIES: (ESSENTIAL FUNCTIONS OF THE JOB) Audit patient charts; review patient's medical history and provide recommendations to providers for closing gaps in supplemental screenings as required by Medicare Quality Programs. This position will create and analyze various metric reports abstracted from the electronic medical record and other medical database systems and applications. Conduct in-bound and out-bound calls to patients including pro-active scheduling of appointments and screenings, reminder calls, and provision of pre-appointment instructions for upcoming medical procedures/screenings. Assembles and mails various educational materials, medical supplies, and instructions to patients upon direction from the provider. Monitor clinical workflow efficiency within the medical clinic setting in order to make recommendations to provider and the care team regarding process improvement opportunities for closing care gaps; and methods for reaching patient care related goals and metrics. Provide continuous education and training to assigned providers and care teams. Contribute to the design and development of educational documents. Initiate, organize and may lead educational sessions for the care team. May provide instruction on Medicare program requirements; Risk Adjustment compliance and proper medical coding. Provide clerical and/or administrative support to clinical staff and practice managers for special studies, projects and reports related to Medicare programs. This position is primarily administrative in nature and does not have accountability for providing direct patient care in the clinic setting REV 3/20/19 QUALIFICATIONS REQUIRED: MINIMUM QUALIFICATIONS: Education: High School Diploma or GED Licensure: LPN Certification: BLS- American Heart Association Work Experience: Three (3) years in a physician clinic or practice environment with intermediate knowledge of front and back office operations. Specific Knowledge/Skills: Knowledge of HCC hierarchy; medical terminology and procedures, ICD-10 and CPT coding, insurance processes. Specific Knowledge/Skills: MS Office literacy including Intermediate Excel skills; electronic medical database applications.
