Job Description

The HIM Specialist II serves as the primary point of contact for the processing of the legal medical record and acts as the custodian of the legal medical record in the daily processing of patient information. This position works with patients, families, providers, clinical and non-clinical staff coordinating the patient's experience throughout the process of accessing health information in interviews with patients by telephone or in person to effectively coordinate patient access to their personal medical information.

Operational Tasks:

  • This position is responsible for the daily processing of patient medical record documentation from nursing units and clinic partners for chart validation and scanning into the legal medical record

  • For analysis of documentation to assure the record is complete through quantitative and qualitative analysis as defined by Centers for Medicare/Medicaid Services / Joint Commission and the El Camino Hospital Medical Staff Bylaws

  • Assigns relevant deficiencies to practitioners for chart completion and to support Title 22 requirements

  • In tandem with the medical staff office, performs physician suspension notifications and acts as a physician liaison for record completion

  • Catalogues paper medical records for off-site storage requests and returns medical records from/to off-site storage

  • Releases protected health information according to HIM policies and procedures and state and federal regulations for patients, continuity of care, legal purposes, disability or denied claims, insurance audits, and HEDIS requests

  • Accesses the third party transcription system to process and electronically submit reports into the medical record

  • Functions as the hospital birth recorder, electronically submitting birth certificate information utilizing the state electronic birth recording system (EBRS) in compliance with Santa Clara County procedures and regulations to include interviewing parents for required information, abstracting clinical indicators for core measures (CQMCC) and obtaining required signatures of parents

  • Validates and approves adult/minor proxy myCare access and provides help desk technical support for patient portal issues

  • Performs Identity functions to include complex chart corrections, erroneous registrations and duplicate medical records for merging to maintain an accurate master patient index (MPI) database

  • Acts as coding liaison supporting the revenue cycle process, functioning as coding operations support and provider relations

  • Supports the clinical documentation improvement (CDI) department with timely provider notification for completion of queries

  • This job functions enterprise wide and includes support and service to clinic and community connect partners

  • Performs other duties as assigned.

Customer Service:

  • Performs validation and registration for access to the patient portal to include providing tutorial instruction to navigate the patient portal software.

  • Answers incoming phone calls, makes arrangements for release of information and birth recording requests including in-person and remote access, monitors access issues with patient portal impacting patient satisfaction.

  • Follows established parameters to ensure state and federal guidelines to assist with timely patient access to health information for continuity of care.

Patient Satisfaction:

  • This position is a key contributor to patient satisfaction initiatives within the HIM department by participating in related process improvement activities as well as providing a high quality contact experience for the patient with every interaction.

  • Creates a positive patient experience by facilitating access to personal health information and submission of required authorization / forms at point of contact. This process gives the patient the appropriate instructions pertinent to their request for information/services.

The HIM Specialist II will be required to work occasional weekends and holidays as scheduled. May be required to work alternate shifts / alternate campuses and others duties as assigned to meet operational needs of the department.

Qualifications

  • High school diploma or equivalent. Associates degree or partial completion at an accredited health information technology program preferred.

  • Intermediate computer skills with Microsoft Office and comfort navigating multiple EMR applications simultaneously

  • Minimum of two years' experience performing in an acute care facility or other comparable healthcare environment which includes chart analysis, birth recording and/or release of information

  • Knowledge of basic medical terminology.

  • Excellent customer service with courteous and pleasant communication skills

  • Demonstrated ability to handle multiple tasks and interruptions.

  • Demonstrated analytical and problem solving skills.

  • Attention to detail and accuracy differentiating text and numeric sequences.

  • Work in a fast pace environment, maintain a high concentration level, and work independently in a team environment with a positive, open-mind focused on continuous improvement

Salary Range:

$26.96 - $40.44 USD Hourly
