Position Objective:

The HIM Documentation Specialist prepares scans, performs quality analysis and uploads scanned images into the Electronic Medical Record (EMR). This position is responsible for maintaining and updating the database which is utilized in the evaluation of physician performance regarding the timely completion of medical records. Performs other clerical duties as required and maintains the physical environment of the area.

Patient Population Served:

This position does not provide direct patient care.

Essential Position Functions:

Performs a variety of tasks related to the conversion of paper records to computer readable images including document preparation, scanning, image cleanup, data verification, and retrieval of documents.

Reviews the accuracy and clarity of the images for quality control purposes.

Utilizes the Electronic Medical Record (EMR) software applications in its entirety.

Retrieves and delivers records upon request to the appropriate parties.

Copies and scans pertinent portions of medical records upon request or as required.

Maintains the designated file area.

Coordinates completion of all birth certificates to meet the State of NH Bureau of Vital Statistics reporting requirements.

Retrieves records from the previous days' discharges from the nursing units.

Identifies outstanding transcription via ChartScript.

Actively contributes to the morale and teamwork of the staff and facility and always presenting a positive attitude and customer-minded vision, with customer satisfaction as the continuing goal.

Maintains the confidentiality of all patient, physician, and hospital-related information.

Provides polite and courteous service to all customers.

Non-Essential Position Functions:

Performs other duties as assigned or as judgment or necessity dictates.

Position Qualifications:

High School diploma or equivalent.
1 -3 years of computer data entry experience required.

Familiarity with scanning software/equipment required.

Strong computer skills including familiarity with MS Word, Excel; experience working with EMRs preferred.

Excellent verbal and written skills.

Familiarity with HIPAA regulations.

Demonstrates an ability to assume responsibility without direct supervision, an ability to set priorities, to exercise initiative and judgment, and to make decisions within the scope of assigned responsibility.

Ability to organize work priorities and meet specific objectives under time constraints.

Ability to manage multiple tasks simultaneously.

Good problem-solving skills and attention to detail.

Ability to be a team player in a team oriented environment and remain calm in demanding and stressful situations.

Tests specimens completely and accurately within established timeframes and according to procedures. Has a comprehensive understanding of laboratory testing, organization, efficiency, quality control, regulatory requirements and the general functioning of laboratory service. Exhibits excellent communication skills, is able to problem solve effectively and communicate solutions to employees in a comprehensive and appropriate manner. Assists Director, Laboratory Services with laboratory problems, solutions to problems, decisions and managerial tasks as needed. These may include interviewing candidates for job positions, payroll, quality management, employee issues and disciplinary actions. Assumes extra responsibilities as needed or requested by the Director, Laboratory Services including training new personnel, ordering supplies, problem solving, assisting and guiding less experienced employees with tasks/problems, and attending hospital meetings. Works closely with the Phlebotomy Supervisor in handling problems and issues that arise. Handles scheduling issues or employee issues that arise in the absence of the Phlebotomy Supervisor. Patient Population Served: This position is expected to demonstrate the knowledge and skills necessary to perform laboratory testing for Frisbie Memorial Hospital patients to include pediatric, adult and geriatric populations. Essential Position Functions: 1. Technical: Performs complex laboratory procedures; recognizes deviation from expected results; analyzes and corrects problems. Verifies handwritten name on test specimen with patient label and type of test to be performed as indicated on the computer-generated label. Performs instrument QC as required. Adheres to established quality control policies and accurately troubleshoots when quality control is unacceptable. Participate in Quality Improvement activities. Evaluates and makes sure test results correlate with diagnosis and each other and understands age specific normal values. Demonstrates the ability to learn and apply new methodologies. Has complete understanding of procedures as outlined in the department policy and procedure manual. Develops the skills and knowledge needed to assist in various departments of the laboratory. Trains and orients new employees and other laboratory personnel as requested. Instructs hospital and other non-laboratory personnel on the proper lab procedures and policies. Prepares specimens and routes them to the appropriate reference laboratory for testing not performed on site. Performs scheduled instrument maintenance procedures. Performs venipuncture and capillary procedures accurately as requested. Orders and restocks supplies as needed. May be required to lift heavy objects. 2. Computer/Clerical: Records and releases results accurately and clearly. Understands the laboratory computer system and uses it effectively, performing all tasks required. Demonstrates keyboarding skills and computer skills. Accessions specimens accurately, comparing patient information provided on the requisition with information on specimens received/processed, assuring consistency and accuracy. 3. Problem Solving: Evaluates and solves problems related to collection and processing of specimens for analysis. Demonstrates the ability to solve problems as they arise and refers problems they are unable to solve to appropriate personnel. Troubleshoots instrumentation and calls the hotline when necessary. 4. Communication/Patient Service: Communicates with staff and/or physicians concerning problem test results. Reports to Director, Laboratory Services any suggestion or positive changes/recommendations within the lab. Greets and communicates with patients in a courteous and professional manner. Displays professional and helpful attitude in resolving client/department problems. Responds to patients and co-workers with empathy and professionalism; provides direction and assistance in a courteous and positive manner. 5. Work Ethic/Professionalism: Strives for self-improvement; keeps up-to-date with department and policy changes and attends appropriate seminars and/or workshops. Maintains patient confidentiality at all times. Promotes teamwork in the department. Completes all work during the shift without sacrificing quality of work. Deals with any issues that require immediate attention in the absence of the Director, Laboratory Services. Reports to Director, Laboratory Services any violations of policies, procedures and standards in compliance with regulatory requirements and hospital administrative policy. Ensures staff is updated and informed of any changes/information, and conducts meetings if necessary. Supports and assists employees as needed. Conduct or assigns training and orientation of new employees. Maintains the department in accordance with regulatory requirements.. Uses good vision in the Lab and looks ahead to problems and recommends changes in a preventive and pro-active manner. Addresses job related errors as they pertain to employees in the Lab. Reviews monthly charges for the Lab assuring no errors in orders or charges. Is pro-active in exploring and recommending changes in the Lab in a preventive and proactive manner. Keeps current with new instrumentation and methodologies. Clearly communicates all concerns/issues related to Lab to Director, Laboratory Services in timely manner. Participates in on-site and off-site CAP inspections. Achieves desired outcomes for all projects assigned by the lab director within established timeframes. Represents Lab at various meetings relative to the Lab (e.g. transfusion committee, Red Cross, Infection Control, LRN, Pathology, etc.). Ensures safety policies are followed. Performs all other duties as assigned by the Director, Laboratory Services. Assumes responsibility for the Lab in the absence of the Director, Laboratory Services. Assists Director, Laboratory Services in performing various assigned responsibilities. (i.e. compliance, troubleshooting, workflow, Meditech Management reports to include batch rejections/billing/variances etc.) Processes payroll in conjunction with or in the absence of the Director, Laboratory Services. Attends hospital meetings as directed by the Director, Laboratory Services. Works closely with the Section Leads in dealing with employee issues or departmental problems. Handles scheduling or employee issues as needed. Performs staff evaluations; 90 day, mid-year and annual. Encourages employees to assist others when short staffed and to work together when required and supports/assists employees as needed. Participates in the interview process for new applicants. Handles disciplinary action of employees to include verbal and written counseling, and termination if necessary. Handles billing issues as they arise. Performs all other similar or related duties as directed or as responsibilities necessitates. Position Qualifications: Bachelor of Science degree in Medical Technology/Medical Laboratory Science or related field; MT(ASCP) and MLS(ASCP) eligible or equivalent for Technologist. At least four years experience in clinical lab. Prior supervisory/management experience preferred. General learning ability needed to learn, understand and apply knowledge in Biology, Chemistry and other clinical lab specialties needed to perform standardized tests on blood, urine and other body fluids, using medical reagent kits and instrumentation that identifies the presence of disease. Verbal ability needed to read and understand written procedures that explain testing procedures used to properly perform each task. Eye-hand coordination and finger dexterity needed to operate specific pipetting and other equipment, and to input test result data into computer.
