This position for use in Health Information Departments ONLY

This position provides entry level inpatient coding support in Health Information Management department. This position works in conjunction with Health Information Coding Educator to complete all applicable educational modules, including didactic and practical experience, with the purpose of developing proficiency with inpatient coding guidelines, other regulatory requirements, and provider queries.

At the conclusion of each educational module, the incumbent will demonstrate proficiency in the following

Thorough review of inpatient records.

Accurate abstract of pertinent data elements from the patient record.

Accurate assignment of International Classification of Diseases, Ninth Revision, Clinical Modification

ICD-9) diagnoses and procedure codes, per Official Coding Guidelines and HIM Policies

Procedures.

Responsibilities

  • Reviews medical records and assigns accurate ICD-9 diagnoses and procedure codes for progressively complex cases.

  • Accurately abstracts pertinent data from the patient health records at greater or equal to 95% accuracy.

  • Sustains greater than or equal to 95% accuracy upon validation on each service line.

  • Achieves and sustains acceptable productivity rate as defined by Coding Leadership.

  • Ensures adherence to Health Information Coding policies and Official Coding Guidelines as published by CMS and Cooperating Parties and as demonstrated by completion of AHA Coding Clinic quizzes, educational material as assigned, and other in-services scheduled by Coding Leadership.

  • Obtains appropriate number of continuing education units in order to maintain credential.

  • Utilizes coding provider query process to request additional information for incomplete, vague, or ambiguous documentation.

  • Completes learning modules, as assigned and in a timely fashion.

  • Participates in performance improvement initiatives, as assigned.

Knowledge / Skills / Abilities

  • Knowledge of medical terminology, anatomy and physiology and pathophysiology.

  • Knowledge of basic coding conventions and use of coding nomenclature.

  • Demonstrated acceptable progression through service lines.

  • Ability to assimilate educational material and demonstrate understanding in each specialty area, per curriculum provided by Coding Educator.

  • Proficiency with healthcare IT systems, preferably Epic, PowerChart, 3M CRS, and Microsoft.

  • Ability to abide by hospital, state, and federal coding guidelines.

  • Ability to be a team player, and to collaborate with colleagues and leadership.

Qualifications

Qualifications Required

  • Must possess at least one of the following certifications: Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT), Certified Coding Specialist (CCS), Certified Professional Coder (CPC), Certified Professional Coder - Hospital (CPC-H), Certified Coding Associate (CCA).

  • Two years of experience with HCPCS, including modifiers and APCs.
