Job Summary The Health Information Management Clerk is responsible for retrieving and reconciling medical records and loose documentation from ancillary departments and nursing units. Other duties may include routine data entry of unbilled reason codes (URCs), timely scanning and indexing of late loose documentation, assisting with release of information (ROI) requests, answering phones, processing accounts assigned to the facility's Horizon Patient Folder (HPF)/ McKesson Patient Folder (MPF) work queues and processing HIM mail. This position may work a day or night shift, and may work with limited supervision, depending upon the staffing needs of the facility HIM department.

Duties (included but not limited to):

  • Retrieves discharged medical records from various nursing units and ancillary departments through the hospital and reconciles them using MEDITECH compiled reports and/or automated reconciliation to ensure that all records are accounted for and ready by the deadline for daily pickup by the courier.

  • Places reconciled records in the designated courier bins for the HSC courier to retrieve and deliver to HSC Document Imaging.

  • Batches, labels and routes loose reports, late charts and/or late documentation to HSC Document Imaging.

  • Scans and indexes loose reports/documents that are identified as critical to coding and prioritize them according to policy.

  • May perform routine data entry to assist in updating applicable unbilled reason codes (URCs) into appropriate systems (HPF/MPF, MEDITECH, Reconciliation Tool) if chart not received/reconciled within established timeframes.

  • Coordinates the retrieval and printing of medical records from storage, as well as the storage, archival and record retention of documents and/or other Alternate Media that cannot be scanned into HPF to include monitor strips (e.g. fetal monitor strips, EKGs, etc.).

  • Assists customers with ROI requests, ensuring the validity and authorization of the request, routing toC I O X , and processing urgent requests on a limited basis

  • May assist with the physician suspension process by printing, stuffing and sending out notice letters when directed, making reminder calls, or recording suspension data in MEDITECH.

  • Processes facility specific HIM mail as applicable.

  • May assist with physician questions related to HPF and the physician portal or other issues that may arise.

  • Maintains or exceeds established productivity and quality standards.

EDUCATION

  • High school diploma or equivalent required

EXPERIENCE
1 year experience in an office or hospital environment preferred. Previous experience in the handling of patient health information and/or medical records is strongly preferred.

PHYSICAL DEMANDS/WORKING CONDITIONS

Requires prolonged sitting, some bending, stooping and stretching. Requires eyehand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment.

Requires normal range of hearing and eyesight to record, prepare and communicate appropriate reports. Responsibilities may require lifting papers or boxes up to 50 pounds with assistance on occasion. May be expected to push or pull 50 pounds on occasions. Work is performed in an office environment. Work may be stressful at times. Contact may involve dealing with angry or upset people. Staff must remain flexible and available to provide staffing assistance for any/all disaster or emergency situations.

Job Summary – The Registrar is responsible for timely and accurate patient registration. Interviews patients for all pertinent account information and verifies insurance coverage. Supervisor – Patient Access Manager/Supervisor Supervises – not applicable Duties (included but not limited to): * Interview patients at workstation or at bedside to obtain all necessary account information. Bed-side registration performed utilizing carts/computers on wheels * Follow AIDET guidelines in all interactions with the patient * Provide exemplary Customer Service * Ensure charts are completed and accurate * Verify all insurance and obtain pre-certification/authorization * Calculate and collect patient liability amounts * Ensure that all necessary signatures are obtained for treatments * Answers any questions and explains policies clearly * Process patient charts according to paperwork flow needs and established productivity standards * Welcome patient and family members in a professional manner. Contact the nursing staff for emergency medical needs and answer patient and visitor questions * Interview incoming patients, his/her relatives, or other responsible individuals to obtain identifying and demographical information with insurance and financial information * Assign Insurance Plans (IPlans) accurately * Search MPI completely and assign the correct medical record number. Notify HIM for any duplicate unit numbers * Verify insurance benefits and determines pre-certification/authorization status via on-line or other resources. If pre-certification/authorization/notification of admission is required and has not been obtained, initiate via Passport, on-line or phone call. - Enter all information and authorization/referral numbers into the registration system. * Secure all signatures necessary for treatments, release of medical information, assignment of insurance benefits, and payment of services from legally responsible parties. Obtain copies of necessary identification and insurance cards. * Understand/explain policies regarding services, pricing, insurance billing, and payment of account. Request full or partial payment for services rendered according to collection policies. * Follows all guidelines set forth in the Cash Handling policy. Produce paperwork on each patient for distribution to appropriate departments. * Cross-trained to register patients in any registration area * Work with physician offices and ancillary departments, providing information when necessary or forwarding relevant documents * Scan appropriate order in Front Office Scanning. * Escort patient to his/her destination or refers patient to an available transporter * Activate all pre-registered patients that have reported for service * Meets/exceeds performance expectations and completes work within the required timeframe. * Practice and adhere to the "Code of Conduct" philosophy and "Mission and Value Statement" * Other duties as assigned KNOWLEDGE, SKILLS & ABILITIES * Communication - communicates clearly and concisely, verbally and in writing.
