Schedule: Monday

  • Friday (Full-Time; 8:00 am

  • 5:00 pm)

Reports To: Hepatitis C Program Manager

ROLE SUMMARY: This position is housed in the Central City Concern Pharmacy and will involve the coordination of services patients in the Hepatitis C (Hep C) treatment program, working closely with providers, patients and pharmacy staff. Program Coordinator scope will include a unique combination of administrative, research and patient care services. This role is especially relevant for individuals pursuing opportunities for direct patient contact, increased knowledge of pharmacy, meeting facilitation, program planning, and leadership opportunities. The Program Coordinator will work closely with partner programs within Central City Concern and in the community, thereby improving health outcomes and patient experience as part of a comprehensive approach to ending homelessness and achieving self-sufficiency.

MINIMUM QUALIFICATIONS (REQUIREMENTS)

  • Required: Valid Pharmacy Technician license, as issued by the State of Oregon's Board of Pharmacy

  • Required: High school diploma (or GED)

  • Required: At least two (2) years of experience required working in community health, including at least one year working with homeless, low income or other vulnerable populations in a supportive role, or working with populations living with HCV/HBV/HIV. A Bachelor's degree in human/social services or closely related field (e.g. social work, public or community health, health sciences, psychology, etc.) may substitute for one year of the required community health experience.

PREFERRED QUALIFICATIONS (ADDITIONAL)

  • Bilingual (e.g. Spanish).

  • Skilled in Microsoft Office Suite, including Excel.

  • Valid driver's license.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Oversee the program application to include but not limited to Hep C medication inventory, patient enrollment, treatment outcome, education and documentation.

  • Collaborate with Hep C team to establish, monitor and achieve goals related to Hep C treatment.

  • Receive, document and direct patient telephone calls in a complete and timely manner according to clinic protocols.

  • Enhance Hep C program quality, assess and improve accessibility of HCV treatment.

  • Collaborate with community partners to advance Hep C program initiatives for CCC clients and others with Hep C.

  • Manage referrals for Hep C services, including referrals for Hep C treatment.

  • Maintain billing and insurance accuracy by performing on-line billing procedures, calling third-party insurers for clarifications of payments, and assisting in preparation, submitting and/or tracking of prior authorization reques

  • Identify criteria to evaluate program outcomes, perform audits, and submit the evaluation to management.

  • Coordinate services with all members of the care team and the client's support network.

  • Facilitate and/or record minutes for scheduled Hep C planning, committee and research meetings and schedule

  • Create organizational systems for Hep C programming, including the creation of policies and procedures as need

  • Quality Improvement and Reporting: Coordinate with Pharmacy Clinical Services Manager, IT, and Quality Management to implement and maintain Hep C treatment program d Make recommendations from the analysis of this data. Track and report data related to patient program admission, progress, discharge and other criteria as needed.

  • Care Coordination and Communication: Work collaboratively with the Hep C treatment team to identify potential barriers for accessing services. Participate in Outreach activities necessary to offer services and encourage participation in a non-coercive manner. This includes in-person and phone outreach to HCV program patients who are due for labs, medication dispensing, and patients who have missed appointments. Coordinate dispensing schedules of prescriptions and monitor necessary lab work and orders. Update Old Town Clinic and pharmacy staff of administrative or workflow changes as directed by the Director of Pharmacy and/or Clinical Pharmacist Lead.

  • Documentation: Work closely with the Medical Records Department to electronically file and index clinical information into the patient's chart in the Electronic Health Record (EHR), as well as obtain, process, and document all necessary information and paperwork required to assist in the provision of HCV services to Old Town Clinic patien

  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and securit Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

PHARMACY-SPECIFIC:

  • Perform all duties necessary to ensure the safe and accurate preparation ofmedications, including gathering necessary patient information, entering it into the computer system, counting or pouring medications and labeling containers.

  • Manages the technician operations of the Pharmacy in a cost-effective manner, including monitoring of inventory and pricing, supporting day-to-day operations, delegating responsibility as needed, and assuring adequate resources are available to meet customer needs.

  • Actively contributes to the planning and achievement of Pharmacy Departmentobjectives and goals.

  • Facilitates the development and implementation of new services.

  • Actively contributes to the evaluation and continuous improvement of patientservices, policies and procedures

  • Works in collaboration with the Hep C pharmacists to develop and meet departmental goals and objectives.

  • Accurately and efficiently utilize GSL, dispensing system, and EH

DESCRIPTION OF OTHER DUTIES:

    1. Provide in-person or telephone follow-up contact to patients identified by Central City Concern for support and management of health-related issue


    1. Collect information regarding patient health history, current health behaviors, and bio/psycho/social needs through a combination of direct interviewing, records review, and consultation with outside sources (after obtaining written consent).


    1. Provide educational information and materials to patients regarding available services in the community, as well as how to appropriately access them, under the direction of the Pharmacy Clinical Services Manager.


    1. Create administrative materials as directed by the Pharmacy Clinical Services Manager.


    1. Perform other duties as assigned by Pharmacy Clinical Services Manager, Sr Director of Pharmacy and/or other Clinical Leadership.
