Now hiring a HEDIS-HCC Outreach Interventionist

Summary:This position is responsible for assisting in closing gaps in care for Medicare STARS and closing gaps in Medicare HCCs through measure-specific outreach. Mainly, HEDIS and HCC outreach is done telephonically by outreach to members to schedule home bone density screenings and annual health risk assessments. Position will perform bone density screenings at members home and deliver cued i-Pads to the members home to complete annual health risk assessments. Reports to Chief Actuary or leadership reporting to Chief Actuary.

Qualifications

  • High School Diploma

  • Medical office experience 1-2 years

  • Knowledge of related medical terminology

  • Excellent driving record

  • Preferred:

  • Associate Degree

  • Bilingual (English/Spanish)

Responsibilities

  • This position includes the following tasks/functions:

  • Manage member-specific lists of members for outreach.

  • Document daily work into EPIC/AHWA/Excel tracking systems and outbound/inbound calls and appointments made and completed.

  • Educate members about getting bone density screenings and annual health risk assessments and provide appointment scheduling.

  • Communicate/engage with providers as needed.

  • Perform bone density screenings at members home and deliver cued i-Pads to the members home to complete annual health risk assessments

  • Organize and distribute test results to Quality PHP Performance Improvement Department.

  • Attend and collaborate with team on community events.

  • Work with peers, leadership and other departments to optimize closures in gaps of care and HCCs. Provide leadership in developing and implementing marketing of deliverable services.

  • Follow HEDIS and CMS technical and compliance specifications.

  • Utilize standard office equipment and field equipment, such as, i-Phones, i-Pads and bone density scanners.

  • Other duties as assigned (may include functions such as administrative support for Risk Adjustment Department).Job Requirements

  • 3-4 hours of phone contact daily

  • Effective time management

  • Logistical/Technical trouble-shooting

  • Track daily work electronically

  • Represent department at community events as needed

  • Understand and implement department procedures

  • Track and measure outcomes of member contact/education

  • Use appropriate office application in support of all activities

  • Must provide own vehicle and related insurance

  • CPR training

  • Performs other functions as required
