Heart Team Coordinator-(Ft, 80%,Days)-Cardiothoracic And Vascular

The Heart Team Coordinator works is responsible for the planning, organizing and oversight of all patients seeking care within the Heart Program. This position works collaboratively with physicians and the healthcare team to guide patients through the clinical management process. The coordinator provides continued education and promotion of the program acting as a liaison in the health care system and in the community.

Required Educational Degree:

Graduate of an Accredited Program in Nursing

Major/Area of Concentration:

Nursing

Preferred Education:

Bachelor's Degree in Nursing (BSN)

Required Certifications:

RN - Registered Nurse with Current Ohio License

Required Experience:

3-5 years of Nursing experience

Preferred Experience:

3-5 years of experience in cardiology nursing

Job Specific Essential Functions:

• Communicates effectively and empathetically with patients and their families and all members of the healthcare team.

• Conducts chart review to identify all needed/required health maintenance, testing and imaging is ordered and resulted in preparation for procedure.

• Schedules and coordinates the surgical procedure for the heart program in collaboration with the patient, physician and hospital based departments.

• Works collaboratively with providers and the care team to ensure patient adherence to medical plan of care.

• Provides appropriate education and resources to patients, families and clinical staff.

• Supports program by ensuring patient follow up appointments are maintained and clinical data is gathered for clinical outcomes.

• Participates in quality improvement initiatives and may participate in multidisciplinary activities and/or coordinate multidisciplinary conferences.

• Communicates pertinent clinical information to the Physician and other appropriate personnel both verbally and in writing; interpreting and documenting procedure and reports according to protocols and maintaining accurate patient records.
