The Coordinator coordinates all services/systems required for an organized multi-disciplinary team approach to the evaluation, treatment and follow-up of the patient needing HF (Heart Failure), TAVR (Transcatheter aortic valve replacement) and/or ACHD (Adults with Congenital Heart Disease) services. The Coordinator will have responsibilities to heart specialty physicians and/or heart specialty mid-levels (Nurse practitioners and physician assistants). The Coordinator will have an area of specialty, however, will be cross trained to cover responsibilities for all HF, TAVR and ACHD patients. In addition, cross training to cover heart and kidney transplant patients, VAD (Ventricular Assist Device) patients, and Cardiac Rehab patients. Population served may include adults, adolescents, and geriatrics.
MINIMUM POSITION REQUIREMENTS
Graduation from an accredited school of registered nursing.
Current California RN license.
Bachelor's Degree within 3 years of hire.
Basic Life Support (BLS) certification
Recent patient care experience in area of heart specialty required
Knowledge of pathology and physiology related to heart specialty required
Demonstrated participation in a multi-disciplinary team
Skills & Knowledge
Knowledge of adult educational planning, implementation, and evaluation.
Excellent interpersonal and communication skills, both oral and written are required; excellent organizational and time management skills.
Basic computer skills to access, enter, and retrieve data; MS Word and Excel spreadsheets.
Bachelor's Degree preferred
Masters preferred (or equivalent heart specialty clinical experience) and clinical research knowledge, preferably within area of heart specialty (ie, HF, TAVR, ACHD).
Advance Cardiac Life Support (ACLS) certification.
Two (2) years clinical heart specialty experience
