We're looking for someone to join our team as a HCH Supervisor who:

Under the direction of Clinica Sierra Vista's Programs Administrator, the Healthcare for the Homeless (HCH) Program Supervisor is responsible for delivering high-quality results working with multiple grant funded programs. The HCH Program Supervisor provides oversight of day-to-day operations and ensures implementation of programmatic components with all grant funded programs while ensuring fidelity to the program and/or curriculum. The HCH Program Supervisor is the lead on grant report submissions, grant renewals, and in keeping all program requirements and data accurate. The HCH Program Supervisor leads staff meetings, and training, and represents the program in the community when interacting with other agencies and boards related to the program.

Essential Functions:

  • Direct supervision of staff and maintaining standards designed to meet program requirements.

  • Monitoring program implementation for clients by providing direction and consultation for staff

  • In collaboration with staff members and the Programs Administrator, develop, revise, and maintain policies, procedures, and standards for each program in accordance to grant standards.

  • Work closely with the Programs Administrator to monitor the budget, specifically the operating, and supply costs of the Health Care for the Homeless Programs. Place office and medical supply orders as needed.

  • Responsible for ensuring proper program documentation and program reporting requirements.

  • Monitor staff schedules and PTO requests to ensure adequate staff coverage at all times.

  • Perform annual performance evaluations for staff and monitor ongoing staff performance. Provide education, coaching, and mentoring for staff as appropriate.

  • Represent Clinica Siera Vista's Health Care for the Homeless team at external meetings with local social agencies to ensure Clinica has input to local issues. Make presentations as needed to stimulate public awareness about services offered.

  • Build external relationships that lead to opportunities such as additional funding sources.

  • Monitors the completion of data quality.

  • Performs other duties as required.

You'll be successful with the following qualifications:

  • B.A./B.S. in health, social sciences or related field (Masters Degree in Social Services, Administration desirable), or a minimum of 10 years' experience working with the Homeless population.

  • Willing to develop working knowledge of community resources addressing medical, social and housing needs of the homeless population.

  • Experience working with grants and/or budgets.

  • Willingness to work in unorthodox settings with unpredictable patient population.

  • Bi-lingual strongly preferred, but not required.

  • Flexibility to alter the 8:00-5:00 schedule when necessary to meet department/business needs
