Requirements / Qualifications

NOTE Internal applicants are required to submit a Letter of Introduction, an updated Resume and any additional required documents (new or in need of an update) not on file. Substitutes must submit ALL required documents. EXPERIENCE: One (1) year of paid experience performing health related services, public contact in a physician's office, hospital or related setting preferred Training in substance abuse and crisis intervention desired EDUCATION: Certificate Copy (Licensed Vocational Nurse, EMT, Certified Medical Assistant, or Certified Nursing Assistant) preferred but NOT required)

Must include the six (6) items listed below in order to be considered for the position.

• CPR/First Aid Certification (Must have valid CPR & First Aid Certificate)

• Driver's License Copy (Must be valid)

• Letter of Introduction

• Letter(s) of Recommendation (Must Submit Two (2) Letters Dated within the last Twelve (12) months.)

• Proof of HS Graduation (Or equivalent)

• Resume
