ESSENTIAL FUNCTIONS:

Provides technical assistance, consultation, education and instruction to staff and the community

Monitors, tracks, documents and evaluates public health prevention and wellness programs, activities and operations and compiles data for report development

Compiles and analyzes data; conducts research, policy and program analysis; and prepares, drafts and generates reports

Develops and conducts community health programs and coordinates projects with internal and external partners and service providers

Develops partnerships and strategies with community agencies and serves as liaison to the department

Examples of Duties

UNDER ADMINISTRATIVE SUPERVISION

Devises strategies for improved service delivery and communicates with community agencies to solve problems

Monitors service provider performance for compliance with program standards

Evaluates service provider programs for effectiveness, and recommends improvements to enhance efficiency, services and performance

Completes special projects, and prepares reports

Develops and maintains a data collection system to monitor, evaluate and report program progress toward achievement of project goals and objectives

Researches, designs, oversees and analyzes public health program data and policies

Reviews legislation, regulations and directives, and recommends modifications to departmental programs

Identifies strategic programmatic and scientific areas in need of strengthening and new areas for development

Evaluates programs and measures program outcomes

Performs related work

Bachelor's degree

Community Health

Epidemiology

Health Administration

Public Health

Social Science with Research Concentration

Related field

WORK EXPERIENCE

One year of health program policy planning and evaluation experience

EQUIVALENCY

An equivalent combination of education and experience may be substituted

OTHER MINIMUM REQUIREMENTS

Must provide one's own source of transportation

Must work a varied schedule

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Community Harm Reduction

This position will work with community partners to identify, implement and evaluate harm reduction initiatives aimed to improve health outcomes for people who use drugs. Familiarity and comfort with the principles of harm reduction highly desired, as is lived experience of substance use disorder.

Criminal Legal Systems

This position will focus on the intersection of substance use disorder, mental health and the criminal legal system. The position will work with community partners to identify, implement and evaluate substance use and mental health initiatives to improve health outcomes for residents impacted by the criminal legal system. Familiarity with mental health and substance use services is strongly desired, as is lived experience interacting with criminal legal system.

VIEW JOBS 8/7/2022 12:00:00 AM 2022-11-05T00:00 Job Description Candidate preferred in the Western Region of KY * Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. * By providing these services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. - This position will work under the direction of a designated professional. * Conduct home visits with Aetna members with chronic conditions to reinforce following the care plan, adjusting tasks to fit with the member's culture, language, and religion, improve access to care, identify health care needs, and reduce unnecessary hospitalizations. * Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting member's health and treatment plan through various communication channels. * Build and maintain positive working relationships with the Aetna members, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. * Work to reduce cultural and socio-economic barriers between clients and institutions. * Assist with care plan implementation and Aetna member education during in-home visits, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of members. * Assist members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals. * Identify gaps and opportunities to strengthen systems of care within the member's community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs. * Assist with patient medication adherence by: instructing the member on current medication list, reviewing medications with member and assist in obtaining refills. * Teach disease self-management (i.e. nutrition, symptom tracking and reporting). * Accompany members to appointments as needed. * Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner (e.g. progress notes, letters), and other administrative responsibilities as needed. * Travel extensively to client homes, community locations, various agencies, and other outreach destinations. Pay Range The typical pay range for this role is: Minimum: 18.51 Maximum: 33.65 Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location. Required Qualifications * 3-5 years relevant experience. * Experience using a computer including the use of Excel and Electronic Health Records. * Requires the ability to travel to multiple office locations and home locations, occasional weekend hours may be required. * Must possess a CHW certification or be willing to obtain one within 90 days of hire. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions. * Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc. * Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings. * Exceptional organizational and interpersonal skills, with attention to detail required. * Ability to work collaboratively in a team and manage multiple priorities, utilizes effective time management skills, and exercise sound judgment. * Able to influence medical care providers as needed to adjust interventions based on member's cultural, ethnic, or religious preferences. * Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. * Knowledge of formal and informal community agencies and resources. * Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. * Ability to communicate medical information to health care professionals and care coordinators over the telephone. * Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary.
