We are looking for a compassionate, dynamic caring outreach worker to provide services to adults with psychiatric conditions as well complex healthcare needs living in the community.
As a Health Outreach Worker (HOW) you will collaborate with the Clinical Care Manager (CCM) to provide care coordination for One Care enrollees with complex healthcare needs. You will also conduct home and community based visits to clients enrolled in One Care with the intent to provide support and education on enhancing health and wellness self management. Additionally, you will also collaborate with Vinfen's Community Based Flexible Support teams as well One Care Plan's clinical staff to maximize coordination and integration of care.

Vaccine Mandate Policy
