CLASSIFICATION/STATUS: Non-Exempt, Full-time, Permanent

IMMEDIATE SUPERVISOR: Manager of Health Information

SUPERVISORY RESPONSIBILITIES: None

PAY RANGE: Band 2 ($18.00-$25.00/hour)

As part of a dynamic team at Charles River Community Health (CRCH), a healthcare practice providing comprehensive and compassionate care to underserved communities, the Health Information Technician organizes and evaluates patients health information and records. In addition, they perform a variety of record keeping and clerical duties in support of services provided in an outpatient, as well as other duties as assigned by the Manager of Health Information.

YOUR RESPONSIBILITIES

  • Generate and prepare/assemble medical records as requested.

  • Organize and evaluate medical records for completeness and accuracy.

  • Retrieve and route records to appropriate personnel or department.

  • Retrieve charts as needed for special audits and peer review.

  • Prepare reports and forms as directed and in accordance with established policies.

  • Maintain and search computerized medical records.

  • Knowledge of chart control, access and storage in accordance with established policies and regulations.

  • Knowledge of medical records regulations and release of health information.

  • Prepare copy of records when presented with properly completed medical release forms.

  • Consult manager, lead, other team members, and appropriate resources to solve health information questions.

  • Receive incoming correspondence and review/forward to appropriate department or staff.

  • Receive, screen and coordinate telephone calls from patients and healthcare providers.

  • Communicate with medical staff, health care team members and patient departments.

  • Perform scanning and uploading of medical charts.

  • Perform a variety of administrative duties including but not limited to: answering phones; faxing and filing of confidential documents; and basic Internet and email utilization.

  • Cross train and assist as requested in other parts of the department functions.

  • Provide excellent customer service to all internal and external customers.

  • Attend mandatory Epic trainings and assessments as needed.

  • Work on Epic In Basket, Overlays, Duplicate Charts, ROI work queues and related work queues.

  • Perform chart corrections, including erroneous encounters, noted and merging duplicate charts.

  • Work on Interface Errors & Incoming Messages/No-linked patients errors.

  • Attend CTC HIM workgroup meetings as required.

  • Other duties as assigned by the Manager of Health Information.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • High School Diploma or equivalent required. Associate Degree and/or certificate program preferred.

  • Bilingual: English/Spanish speaking preferred.

  • At least one year of experience within the last three years in a comparable job classification required.

  • Knowledgeable about and compliant with HIPAA regulations; maintains strict confidentiality of client information.

  • Complies with Infection Control, Standard Precautions and OSHA standards for the healthcare professional.

  • Able to wear personal protective equipment (e.g., face mask, goggles, latex/non-latex gloves) as needed.

  • Ability to work effectively within role independently and with other team members required.

  • Ability to organize and complete work in a timely manner required.

  • Ability to read, write and effectively communicate in English required.

  • Ability to understand medical/surgical terminology required.

  • Demonstrated interpersonal communication skills required.

  • Demonstrated analytic and problem solving skills required.

  • Must be self-reflective, open to feedback, and able alter communication style based on the audience.

  • Must be hands-on, serving as a role model for others in the department.

  • Must be able to work in an environment where there are constant interruptions.

  • High level of integrity and transparency required.

  • Experience in diverse culture preferred.

  • Physical demands: stooping, turning, bending, squatting, kneeling and the ability to lift up to 10 pounds; constant/repetitive standing; requires normal, correctable vision and hearing, and the ability to accurately discern color as necessary to perform job functions.
