The Health Information Specialist will report to the Health Information Management (HIM) Manager. The Health Information Specialist works within prescribed procedures to ensure secure handling and processing of medical records. The HIM Specialist may assist with record collection, prepping, scanning, indexing, analysis, reanalysis, and release of information.

They will be using St. Vincent Health (SVH) / Health Information Management (HIM) analysis guidelines for required documentation to determine whether or not records are complete. This person will also assist physicians by answering questions regarding completeness of the electronic medical record. The Health Information Specialist will perform audits of records for requirements for Joint Commission Accreditation and Conditions of Participation.
Your daily duties may include:
Maintaining patient confidentiality
Prioritizing work based on established time frames
Retrieving medical records from defined locations
Checking medical records into the HIM department and identifying and following up with missing charts
Preparing the medical record for scanning following prepping guidelines
Scanning documents into the medical record imaging system and providing necessary scanner maintenance
Performing quality control checks on scanned medical records and correcting errors
Indexing / validating documents into medical record imaging system
Following SVH and HIS guidelines to analyze inpatient and outpatient records for accuracy and completeness using the electronic record system
Entering all deficiencies into the electronic record system
Tracking and trending physician deficiency and delinquency data for reporting to HIM leadership
Communicating with physicians daily as questions arise regarding the completeness of the medical record
Collecting data for medical record audits and HIM process improvement activities
Actively participating in physician on-boarding and assisting physicians as needed
Providing assistance to internal and external customers
Monitoring the ROI work queue/faxes/and mail requests
Processing walk-ins/phone calls/ROI requests
Managing transcription support desk and resolving transcription issues
Maintaining and upgrading the knowledge and skills affecting the quality of the work done
Adhering to the organization's corporate complianceprogram; adherence includes but is not limited to attendance at mandatory
Corporate Compliance in-services, carrying out position responsibilities consistent with program guidelines, and reporting any violation of Corporate Compliance policies to your supervisor
Performing clerical duties as assigned
Routing any problems or inaccuracies to the appropriate workbasket for correction and/or reassignment
Performing record reviews relative to Joint Commission guidelines as well as Conditions of Participation
Practicing service excellence through communication skills and create a healthy work environment by treating others respectfully
Managing conflict by identifying solutions or seeking guidance as necessary
Maintaining the flexibility required in order to provide services as assigned to alternate work sites
High School diploma
Good customer service skills and an aptitude for teamwork
Knowledge and organizational ability to review medical records for completeness byfollowing HIS guidelines
Ability to learn medical terminology
Ability to enter data at an acceptable level of speed and accuracy and to operate a computer using current Health Information Systems
Ability to pay close attention to detail to detect missing and/or incorrect information in the medical record and must also have the knowledge and skills needed to make the corrections when appropriate
Ability to multitask.
Ability to maintain production standards as set forth by leadership.
