About the Job

• Your Role

In this position you will coordinate the activities of Medical Record Services, including all program services provided by the Health Services Division. You will be responsible for planning, organizing, and coordinating the utilization review (UR), information services (IS), and patient records. These activities must meet all the requirements set forth by Administrative Rules and federal and state laws/requirements pertaining to medical information. You will also serve as custodian of all adults in custody's medical records maintained by the Health Services Division.

Processes, maintains, compiles, or reports patient information for health requirements or standards in a manner consistent with the healthcare industry. Protects the security of medical records to ensure that confidentiality is always maintained.

Responsibilities include but are not limited to:

• Utilization review in the activities of review and analysis of medical records, which ensures consistency, completeness, and accuracy of documentation.

• Obtaining, analyzing, and reviewing medical records of inpatients and short stay patients following discharge from the hospital.

• Verifying and entering patient information into the system.

• Analyzing medical records for diagnoses, procedures, and diagnostic related groups (DRG).

• Maintaining a tracking and recall system.

• Assisting in developing and maintaining current written policies and procedures that govern the maintenance of quality patient records which are completed in a timely manner.

• Responding to subpoenas for patient records as required by state and federal law. This includes presenting medical records in court hearings and copying records when appropriate.

• Preparation and maintenance of patient records either by establishing new records, filing reports, reviewing charts after appointments for deficiencies and pulling charts as necessary.

• Assisting in medical terminology and application of diagnosis coding.

• Retrieve patient medical records for physicians or other medical personnel

• Resolve or clarify codes or diagnoses with conflicting, missing, or unclear information by consulting with physicians or other medical personnel

• Contact a medical profession to obtain information or signatures

• Review or analyze medical records for completeness, accuracy, or compliance with regulations.

• Process patient admission or discharges documents.

• Plan, developed, maintain, or operate a variety of health records indexes or storage and retrieval systems to collects, classify, store or analyze information

This position is represented by Association of Oregon Corrections Employees, (AOCE). This position may be eligible for Police & Fire (P&F) benefits through Public Employees Retirement System (PERS), as defined by statute.

Minimum Qualifications

Two years of experience related to the class concept.

OR

An Associate's degree in Health Information Technology from an accredited program.

OR

An equivalent combination of training and experience.

Working Conditions

You will be expected to role model prosocial behavior and have an attitude conveying dignity and respect in the treatment of others. Able to acknowledge everyone is capable of positive change. Requires contact with adults in custody who may become violent. Walking or standing for long periods of time, climbing and descending stairs. Exposed to chemicals, radiation, communicable diseases, or other hazards inherent in a health care and correctional setting. You will need to be able to lift 50 lbs., maneuver, and move with or without reasonable accommodations.

The salary range listed above is for non-PERS qualifying applicants. PERS qualifying applicants may be eligible for an adjusted range to reflect an additional 6.95%.

Requirements for Employment

The successful candidate for this position must pass a Criminal Justice Information Systems (CJIS) fingerprint-based background check and maintain CJIS eligibility. Due to CJIS requirements related to system access, there may be some felony convictions that could disqualify you for this position.
