The Health Information Clerk performs a variety of activities for the maintenance and control of the medical record to ensure accuracy and compliance in accordance with all regulatory, licensing requirements and standards
ESSENTIAL FUNCTIONS (include, but not limited to the following)
Reviews and analyzes medical records for organization and consistency of documentation to ensure compliance with established regulatory and licensing requirements
Receives and responds to requests for release of confidential medical records information; verifies confirmation of release authorization
Researches missing documentation, records or materials through review of departmental documents and follow up with departments and/or physicians as appropriate.
On daily basis, reviews EHR reports for missing documentation and forwards request for completion to appropriate care provider.
Scans incoming paper based material into EHR.
Purges and forwards inactive records to storage center in accordance with established procedures
Responds to requests for medical record review by retrieving and forwarding charts.
Provides regular productivity reports as required.
Participates in departmental orientation, on the job training and quality assurance programs/initiatives.
Participates in a variety of department and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Supervisor
Performs other duties as assigned.
Six months of previous related experience in an office or healthcare environment, required
Experience with entry and research of patient health information in a computerized system, preferred
High School Diploma or GED equivalent, required
