GENERAL SUMMARY

Performs clerical functions relating to reviewing and maintaining medical records for consumers and preparing documents to be sent to Central for scanning.

ESSENTIAL DUTIES & RESPONSIBILITIES

Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation.   • Creates, maintains, scans, and reviews consumer medical records, documents, reports, and information for the program.

  • Sorts, assembles, and preps consumer documents for filing into paper consumer records or for scanning into the electronic medical record.

  • Requests consumer documents from external health care providers and hospitals.

  • Collects request forms for release of information and forwards to Central release of information department for processing.

  • Reviews consumer documents to ensure completion. Reports deficiencies to supervisor for follow up and compiles related deficiency report.

  • Assists internal and external customers with health information inquiries.

  • Prepares paper records (not scanned) for transfer to offsite storage in accordance with established procedure.

  • Performs other related duties as required.

MINIMUM ENTRANCE QUALIFICATIONS

Education and Experience

  • High School diploma or equivalency and one (1) year of experience in an office setting.

Licenses or Certifications

  • None

Other Requirements

  • Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business.

  • Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.

  • Maintain accurate and complete records.

  • Maintain the confidentiality of information and professional boundaries.

  • Meet schedules and deadlines of the work.

  • Prepare clear and concise reports.

  • Understand and carry out oral and written directions.
