GENERAL SUMMARY
Performs clerical functions relating to reviewing and maintaining medical records for consumers and preparing documents to be sent to Central for scanning.
ESSENTIAL DUTIES & RESPONSIBILITIES
Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation. Creates, maintains, scans, and reviews consumer medical records, documents, reports, and information for the program.
Sorts, assembles, and preps consumer documents for filing into paper consumer records or for scanning into the electronic medical record.
Requests consumer documents from external health care providers and hospitals.
Collects request forms for release of information and forwards to Central release of information department for processing.
Reviews consumer documents to ensure completion. Reports deficiencies to supervisor for follow up and compiles related deficiency report.
Assists internal and external customers with health information inquiries.
Prepares paper records (not scanned) for transfer to offsite storage in accordance with established procedure.
Performs other related duties as required.

Other Requirements
Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business.
Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.
